An Atypical Cause of Corneal Epithelial Defect-One Case Report

角膜表皮破損的非典型成因-病例報告

摘要


目的:報告一例以直線型態表現的角膜表皮破損。 方法:病例報告。 結果:一位二十三歲無特殊病史的女性因左眼疼痛一個月無法緩解而轉診至本院,檢查發現直線狀角膜破皮及點狀表皮侵蝕。此病人對傳統的人工淚液及自體血清點眼治療反應不佳,在睡眠時間給予眼罩後,角膜表皮破損及點狀表皮侵蝕才獲得改善。 結論:此病例報告的直線狀角膜表皮破損及點狀侵蝕是來自於機械性的摩擦,由於睡眠時間的眼罩配戴治療效果較傳統治療顯著,其角膜破損可能是由一非傳統的病因所造成。

Purpose: To describe the case of a woman who presented with corneal epithelial defect manifested as a pattern of vertical lines that failed to respond to traditional treatment, but responded well to the application of metal shield protection. Method: Observational case report. Results: A 23-year-old woman without specific history presented with pain in the left eye for 1 month. Unhealed corneal epithelial defect associated with punctate epithelial erosions (PEEs) were detected in the left eye. The patient responded poorly to initial treatment with lubrication and autologous serum eyedrops. After wearing a metal shield in the affected eye during sleep, the corneal epithelial defects and PEEs were no longer observed. Conclusion: In this case, the vertical linear epithelial defect associated with PEEs was induced by mechanical abrasion during sleep. Thus, we hypothesize an alternative cause of this defect, separate from other well-known etiologies for superficial corneal erosion, because of the patient's poor response to traditional treatments.
