Clinical Evaluation in a Patient with Retinitis Punctata Albescens

摘要

目的：報告一個白點狀視網膜炎接受詳細臨床評估的少見病例。
方法：病例報告。
結果：一位53歲女性無特別臨床症狀，於門診中意外發現雙眼有許多白點狀物於周邊視網膜，但黃斑部並無異常。經過詳細之臨床檢查，螢光眼底攝影發現於視網膜周邊出現許多點狀高度螢光斑，視網膜電圖分析發現暗視覺的振幅下降，眼電圖表現出Arden氏比例下降，色彩視覺檢查發現錯值偏高，同時暗適應檢查之桿狀細胞閾值偏高，但視野檢查正常。
結論：提供一個白點狀視網膜炎少見之病例，完整的視網膜功能檢查，能有助於正確的診斷此疾病。

Objectives: To present a rare case of retinitis punctata albescens in a 53-year-old woman who subsequently underwent detailed retinal function examinations.
Method: A case report.
Results: A 53-year-old woman without any clinical presentation was incidentally found to have multiple yellowish-white dots scattered over the midperipheral areas of the fundus with macular sparing in both eyes. Detailed retinal function examinations were performed in our clinic. Fluorescein angiography showed multiple midperipheral hyperfluorescent dot lesions. An electroretinogram revealed abnormal scotopic amplitude, and an electrooculogram showed a reduced Arden ratio. Farnsworth-Munsell 100-hue color vision test results showed increased total error scores. Dark adaptometry testing revealed an elevated final rod threshold after 30 minutes of dark adaptation. Goldmann perimetry revealed no significant visual field constriction.
Conclusions: We present a rare case of retinitis punctata albescens. Clinical findings and complete retinal function examinations are important in differential diagnosis.
