摘要
交班是否完整，直接影響護理品質。臨床常出現交班上遺漏，於是本專案針對五家醫院八個內外科病房深入瞭解原因，發現交班完整性與個人學經歷、交班方式、是否有交班規範可依循及交班欲達到的功能有關，進而確立問題、礙定改善方案，包括交班規範的建立、擬定在職教育、交班表格電腦化的設計及評值表的製定，結果顯示交班完整性明顯改普達90%，同時節省每位護埋人員交班時間約15分鐘，可見交班方案擬定確實能改警交班問題，提昇交班品質。

A complete shift related directly to nursing quality. The incomplete nursing shift is a common problem in clinical practice so that the researchers conducted this project. The sample for the project consisted of 8 medical and surgical words in 5 hospitals. The researchers found that the incomplete shift related to nurses education, methods of shift, standard of shift, and function of shift. Therefore, the researchers set the problems and intervention of the project that included standard, inservice education, computerized form, and evaluation of estabilshment in the nursing shift. The findings of the project indicated the 90% improvement in complete nursing shift. At the some time, nurses saved 15 minutes in the shift. Therefore, this project con decrease the problems of nursing shift and devote the quality of nursing.
