護理師常是首位評估急救需求的專業人員,及早提供有效的胸外按壓,可改善病人存活率及有利神經學的預後,當新進護理師執行急救能力不足時,將危及病童的生命,甚至衝擊其對臨床工作的心理調適。藉由事件的分析及品質改善,發現造成急救能力不足的原因包括:缺乏急救經驗,對於急救車用物放置位置及急救藥物的給予劑量不熟悉,急救設備內容物種類多且未落實稽核研究機構院內護理急救技術評核。此次品質改善活動期望兩年期新進護理師之執行急救能力完成率由33.3%提升至86.7%。改善策略包含:實體教育訓練課程、跨團隊合作擬真情境模擬訓練、建置急救用藥稀釋及給藥流程。評值改善策略之成效,兩年期新進護理師執行急救能力完成率由33.3%提升至94.4%,有效提升護理師對於急救流程之認知與技能,有助於個人專業的增長,並增進自我信心,進而提升重症照護品質。
Mostly nurses are the very first expertise to assess the need for emergency care. They could provide effective chest compressions to patients as soon as possible; it can improve patient survival and favorable neurological prognosis. When the rookie nurses perform the first-aid ability unskillful, not only endanger the child's life but also impact their mental accommodation of clinical nursing activity. According to cases study and quality assurance progress, we found unskilled first-aid ability including: lack of first-aid experience, not familiar with the placement of emergency vehicle and the emergency medicine dose, the variety of first-aid equipment, and insufficient carrying out the evaluation of nurses first-aid treatment ability in hospital. The quality assurance progresses are planning two-year duration for rookie nurse division to increase their first-aid treatment completion rate from 33.3% to 86.7%. The quality assurance strategy includes: intermediate training course, multi-disciplinary team work to simulate the reality scenario training, building up emergency drug dilution and drug delivery operation procedure. Evaluation of the effectiveness of the quality of first-aid ability completion rate from 33.3% to 94.4% of new nurses in their two years, enhances the nurse's first-aid domain knowledge and skills, implements self-achievement and realization, and promotes the quality of critical care.
