第二型糖尿病患自我照顧行爲及其相關因素之探討

摘要

本研究的目的在探討第二型糖尿病患自我照顧行為分別與人口學特性、疾病相關特性以及社會支持的關係。採用橫斷式調查研究法，以立意取樣南台灣某醫學中心新陳代謝科門診，40歲以上之第二型糖尿病患131名，採用人口學變項問卷、疾病相關變項問卷、社會支持量表和自我照顧行為量表進行資料收集。研究結果發現：大專程度（含）以上的病患對高低血糖的預防和處理之自我照顧行為最佳。罹患糖尿病越久者，其足部之自我照顧行為越佳。本研究中糖尿病患之自我照顧行為指標分數為74分（滿分100分），屬於中上程度，其中以藥物與血糖監測的自我照顧行為得分最高，運動則得分最低。採用飲食控制合併口服降血糖藥物和規律運動者，以及社會支持程度越高之糖尿病患，不論在整體自我照顧行為或各面向自我照顧行為都是最佳。本研究結果可以提供醫護相關人員作為糖尿病衛生教育介入措施之參考。

The purpose of this study was to explore the relationships between self-care behaviors and the demographic characteristics, disease related characteristics, and social support, respectively, among patients with type 2 diabetes mellitus. With a cross-sectional design, 131 participants with age 40 or above were recruited from outpatient department in a medical center in southern Taiwan. The structured questionnaires, including demographic characteristics, disease related characteristics, social support scale, and diabetes self-care behavior scale were administered. The results showed that participants with junior college or higher educational level had higher scores in preventing high and low blood glucose. The participants with longer period of suffering from diabetes had higher scores in self-care for their feet. The index score of self-care behavior was 74 (full mark:100). It was in a mid-high level. The highest score for self-care behavior was in the 'medication and glucose monitoring' dimension and the lowest score was in the 'exercise' dimension. Participants undertaking on a diet, taking diabetic medicine, and engaging in regular exercise, and those with high level of social support had higher scores in self-care behaviors and its dimensions. The results of this study could be used as a reference for diabetes health education program.
