A Seven-year-old Boy with Traumatic Asphyxia due to a Chest Crushing Injury: A Case Report and Review of the Literature

外傷性窒息並不常見,常因胸部或上腹受壓所致。我們報告一例七歲男童前胸壓傷,於急診呈頭頸部膚色變紫,結膜下出血及臉部水腫。當時意識清楚,脈搏稍快,無呼吸道阻塞現象。電腦斷層有輕微右下肺葉挫傷。住院採保守治療-頭部抬高,氧氣供應。五天後順利出院。單純外傷性窒息並不像第一眼所見的險惡,伴隨傷對於處置及預後相當重要。

Traumatic asphyxia is rare and often associated with chest or upper abdomen crushing. In this report, a seven-year-old boy was crushed over the anterior chest. Violet skin over the head and neck, bilateral subconjunctival hemorrhages and facial edema were observed. Clear consciousness, mild tachycardia and no airway obstruction were found. Only a mild right lower lung contusion was detected by chest CT. He was admitted to SICU for supportive care with head elevation, O2 supplement. He was discharged uneventfully five days later. Traumatic asphyxia may be not as ominous as the patient's first appearance indicates. Associated injuries are often significant in terms of both management and prognosis.
