背景:產後憂鬱不僅影響母親的身心健康,也影響新生兒睡眠與成長,然國內鮮少有關產後憂鬱對母育信心、睡眠品質與嬰兒睡眠品質之相關研究。目的:本研究旨在探討母親產後憂鬱與母育信心、睡眠品質及嬰兒睡眠品質之相關性。方法:採橫斷式相關性研究設計,以方便取樣,針對北部某區域醫院兒科門診個案進行收案,研究工具包括愛丁堡產後憂鬱量表、匹茲堡睡眠品質量表、母育信心量表、簡易嬰幼兒睡眠品質量表,以卡方檢定及獨立樣本t檢定檢測產後母親憂鬱對母育信心、睡眠品質及嬰兒睡眠之影響。結果:共203位個案參與,13.3%母親有產後憂鬱現象,母親有產後憂鬱者,其母育信心(X^2=10.0, p=.002),睡眠品質(X^2=-4.53, p<.001)顯著低於母親無產後憂鬱者,且嬰兒睡眠問題,包括嬰兒夜醒次數(t=-3.29, p=.001)、睡眠潛伏期(t=-2.84, p=.005)及夜醒總時間(t=-3.38, p=.001)顯著高於母親無產後憂鬱者。結論:醫護人員應重視母親產後憂鬱問題及其對母育信心、睡眠品質與嬰兒睡眠問題的影響,並進一步提供介入措施,期能改善產後母親及嬰兒之健康。
Background: Postpartum depression not only affects the mother's physical and mental health, but also affects the sleep and growth of newborns. However, related research is limited. Aims: The study aims to explore the relationship between mothers' postpartum depression and maternal confidence, sleep quality and infants' sleep quality. Methods: This is a cross-sectional study design using purpose sampling. Participants were recruited from the pediatric outpatient department of a regional hospital in Taipei City. Instruments included Edinburgh Postpartum Depression Scale, Pittsburgh Sleep Quality Scale, Maternal Confidence Scale, and Infant Sleep Scale. Data analysis used chi-square test and independent sample t-test to detect the influence of different maternal confidence, sleep quality and infant's sleep on postpartum depression. Results: A total of 203 cases participated. 13.3% of mothers had postpartum depression, and mothers who had postpartum depression had their maternal confidence (X^2 = 10.0 , p < .002). Sleep quality (X^2 = -4.53, p < .001) was significantly lower than that of mothers without postpartum depression. Infants with sleep problems, including the number of times the baby wakes up at night (t = -3.29, p = .001), sleep latency (t = -2.84, p = .005) and total time to wake up at night (t = -3.38, p = .001) were significantly higher than those of mothers without postpartum depression. Conclusion: Medical staff should pay attention to the mother's postpartum depression and its impact on maternal confidence, sleep quality and infant sleep problems, and further provide intervention to improve the health of the postpartum mother and infant.
