吸菸會造成身體多重器官疾病,也會增加癌症及猝死的風險!因此戒菸對於疾病的控制相當重要,改善前若要利用住院期間提供病人戒菸衛教,大多需依賴病房護理人員的轉介,然而目前轉介率極低僅13.17%。本專案目的為提升住院吸菸病人戒菸轉介率達80%,透過制訂戒菸個案轉介機制:(1)優化戒菸轉介系統;(2)舉辦在職教育訓練;(3)戒菸個管師至病房進行菸害防制宣導,跨團隊合作結合工作規範流程及資訊系統,成功的將「自動轉介模式」運用在戒菸病人照護。院內所有住院吸菸病人戒菸轉介率自13.17%提升至100%,且戒菸成功率從34%提升為44.5%,本專案結合資訊系統跨團隊合作,成功的將自動轉介模式運用在戒菸病人照護且成效良好,不但增加服務的廣度也讓護理人員操作系統從12個步驟減至1個步驟,達到事半功倍的效果。
Smoking causes diseases in several organ systems and is a risk factor for cancer and sudden death. Therefore, smoking cessation is vital for the management of chronic disease. Most inpatients rely on referrals from the nursing staff to access the smoking cessation program; however, referral rate was low (13.17%). This project was designed to increase the smoking cessation referral rates to more than 80% for inpatients. To develop a referral mechanism for cases of smoking cessation, we (1) optimized the referral system, (2) held training programs for educators, and (3) had cessation advisors to enter the wards to publicize the smoking cessation program to inpatients in person. This interdisciplinary effort combined process specification and information systems to successfully apply the "automatic referral process" in the care of patients undergoing smoking cessation. The referral rate increased from 13.17% to 100% and the point success rate increased from 34% to 44.5%, respectively. Through the combination of information systems and interdisciplinary collaboration, we successfully applied the automatic referral process to smoking cessation among patients. We increased the breadth of referral services and reduced the operating system of nursing staff from 12 steps to 1 step, which allowed us to do more with less.
