Infarct Volume, Edema and Neurological Score in a Transient Focal Ischemic Rat Stroke Model

恢復時間對梗塞體積、腦水腫及神經症狀之影響 ─ 以大鼠中大腦動脈阻塞為模型

背景與目的:臨床上腦血管梗塞是最常造成中風的原因,藉動物模型,則可更深入探討此類相關問題,以助於臨床之應用。本研究則是以大白鼠中大腦動脈阻塞(Middle Cerebral Artery Occlusion, MCAO)方式做為中風動物模型,探討梗塞後不同恢復時間(24小時或14天)對梗塞體積、腦水腫及神經症狀之影響。方法:24雙大白鼠受右中大腦動脈阻塞手術,阻塞時間為60分鐘,其中12隻經24小時恢復,另12隻經14天恢復後,動物犧牲,取腦以測其梗塞體積及腦水腫大小,並於犧牲前先評估其神經症狀之分數。結果:中大腦動脈阻塞大白鼠經14天之恢復後,其梗塞區域(11.19%±5.38%)小於24小時恢復後之梗塞區域(17.52%±10.97%)(t=-1.79 , P=.1049)。而腦水腫之下降則更為明顯(3.14%±4.60%比較於7.98%±5.20% , t=-2.42 , P=.0306)。結論:根據本研究結果顯示暫時性腦血管阻塞後,腦區域受損範圍與腦水腫現象均與恢復時間長短相關,本研究提供了解中風後隨時間之生理病理變化,並提供未來探討訓練及治療效果之基礎。

Background and Purposes:Survivors of a stroke manifest abrupt development of a neurological deficit. Middle cerebral artery(MCA)occlusion is a widely used experimental model of ischemic stroke. The purposes of the present study were to investigate the effect of recovery time(24 hours versus 14 days)on infarct volume, brain edema and neurological deficits by using the transient rat MCA ischemia model. Methods:Twenty-four young male Sprague-Dawley rats were undergone MCA occlusion procedure for 60 minutes under an operating microscope. Twelve rats were killed 24hours and 12 rats were killed 14 days after the MCA ischemic procedure. The infarct volume, edema and neurological score were measured for comparison between the two groups. Reults: The mean infarct volume measured 24 hours post ischemic procedure was 17.52%±10.97% of total volume of the ipsilateral hemisphere, and was 11.19%±5.38% when measured at killing 14 days post ischemic procedure (t=-1.79 , P=.1049). A significant reduction (P=.0306)in edema had also been noted in rats after 14 days of recovery (3.14%±4.60%)compared to that after 24 hours of recovery(7.98%±5.20%).Conclusion:The decrease of infarct volume at 14 days after the ischemic insult, suggested a resolution of brain edema and possible loss of brain tissue. In conclusion, the present study demonstrated that infarct volume and edema were time dependent. Major advantages of the model used in the study were a 100% incidence of infarction and the availability and low cost of rats. Because the model is reproducible, the effects of various therapeutic agents on neurological outcome and size of infarction produced by focal cerebral ischemia can be studied in the future.
