目的 探討因創傷等引起的急性肺損傷(ALI)和急性呼吸窘綜合徵(ARDS)患者氣道內給予地塞米鬆的治療效果。方法 64例符合ALI/ARDS診斷標準的患者,建立人工氣道,行機械通氣。充分濕化吸痰後,首次氣道內注入10mg地塞米鬆,第1天氣道內注入地塞米鬆5mg/h,第2天注入5mg/2h,第3天注入5mg/4h,3d後逐日減爲2.5mg/4h、2.5mg/8h後停藥;地塞米鬆全身用量爲10~20mg/d,2~3d後停藥。結果 ①呼吸機治療氣道峰值壓力平均降低8.20cmH2O;②動脈氧分壓昇高至61.90~93.00mmHg。結論 氣道內應用地塞米鬆能明顯改善呼吸功能,阻止ALI病程向ARDS的發展。同時由於激素局部用藥,減少大劑量激素用藥不良作用。

English Abstract

Objective To explore the treatment effects of dexamethasone used through inner-trachea on acute lung injury and ARDS caused by trauma. Methods Sixty-four cases were diagnosed as ALI/ARDS and supported by mechanical ventilation. After fully moistness and suction, 10mg dexamethasone were given to the patients through endotracheal route. The dose of dexamethasone was 5mg per hour at first day, 5mg per 2 hours at second day, and 5mg per 4 hours at third day. After three days, the dose of dexamethasone decreased to 2.5mg every 4 hours, 6 hours and 8 hours day by day. 10~20mg dose of dexamethasone was used by venous for 2~3 days. Results ① The peak pressure of airway was decreased, and the mean value was 8.20cmH2O. ② The partial pressure of artery oxygen was raised to 61.90~93mmHg. Conclusion Endotracheal partial dexamethasone can improve the respiratory function and prevent the development of acute lung injury to ARDS. Because of the dexamethasone used in local airway, its side effects are reduced.
