照顧一位腦部腫瘤病患之護理經驗

A Nursing Experience of Caring for a Brain Tumor Patient

摘要

本報告是護理一位50歲男性因左側腦部腫瘤術後併發右側肢體偏癱及失語症而導致病患在面對疾病所引發身體功能喪失、產生身體、心理問題的護理經驗。照護期間，筆者運用會談、觀察、傾聽等技巧，採用Gordon十一項功能性健康型態評估，經分析其主要護理問題爲無力感、言詞溝通障礙、身體活動功能障礙、照顧者角色緊張；依據個案的個別需求，提供（一）正向情緒支持，強化健肢功能，協助個案心理調適並渡過無力感危機（二）利用醫療團隊力量，與個案及家屬共同擬定復健計畫，協助個案能積極參與語言訓練及肢體活動功能訓練（三）教導並提供家屬疾病相關知識及照顧技巧，期待病患在出院後，家屬有足夠能力照顧病患。

This report described a nursing experience when caring for a 50-year-old male with a tumor on the left side of the brain. The patient suffered from postoperative right-side hemiplegia and aphasia, encountering physical and psychological problems as result. During the nursing period (October 1(superscript st) to October 30(superscript th), 2008), the author applied techniques of interview, observation and listening, as well as the Gordon's Functional Health Pattern Assessment to identify nursing care problems, which were powerlessness, communication disorder, impaired physical mobility and caregiver anxiety.
According to the patient's individual needs, nursing care measures then included: (1) providing positive emotional support, improving the functions of healthy limbs, assisting with psychological adjustment, and helping overcome feelings of powerlessness; (2) involving the patient, family, and health care team to develop a collaborative rehabilitation approach that encouraged the patient's active participation in language therapy and physical function improvement training; (3) providing the family disease-related information and teaching caring skills, expecting the family to possess full capability of caring for the patient after discharge.
