長期照護具有長期性與複雜性的特質,多元專業已陸續介入但彼此缺乏整合性的作法。因此,資源管理已經成為長期照護服務或方案成功的關鍵,資源管理的主要目標在於提升資源運用效能,避免資源缺口、資源重疊或錯誤使用而造成浪費,同時亦能創造更具永續發展能力的資源系統。本文介紹社會工作在長期照護領域工作方法由個案工作到個案管理再進一步到資源管理的發展脈絡、理論基礎並提出實務案例討論。文中引Bouwen & Taillieu(2004)之研究,強調多方參與模式是一種社會學習的循環,一方面注重問題或任務議題,另一方面則注重社會關係議題;整體而言,可稱為「關係的實踐」。另外,實務方案設計時要特別重視共同目標的確認。
Long Term Care (LTC) is a complicated and long termed issue. Over the years, multiple disciples have joined the LTC team; however, the cooperation and coordination among team members may require further improvement. Resource management (RM) has become a key to successful LTC services and projects. The purposes of RM are to avoid resource misuse and overlap, to reduce resource waste and to improve the efficiency and effectiveness of resource usage. This article explained the professional development course of social work from case work and case management to resource management. Theory of social learning by Bouwen & Taillieu (2004) was applied to guide the two-thronged approach of LTCRM, i.e., dealing with the social-relational issues and the problem issues in a circular way. Besides the emphasis on relational practice, reaching goal consensus is also very important.
