Ejaculatory Duct Obstruction Combined with Congenital Unilateral Absence of the Vas Deferens
射精管開口阻塞合併先天單側輸精管缺失之治療
摘要
在我們的病例報告中，患者為31歲不孕男性，其配偶為26歲健康女性。在過去兩年間未避孕而仍無法受孕。理學檢查發現其雙側睪丸大小正常，但觸診發現僅左側具輸精管，右側輸精管疑先天缺失。血清學方面，賀爾蒙也無異常現象。精液分析為完全無精蟲。輸精管攝影確認為射精管開口阻塞。患者接受綠光雷射氣化術切開射精管開口。術後兩個月，在精液中發現具有活動力的精蟲。回顧目前的文獻，這是以綠光雷射氣化術治療射精管開口阻塞的第一例。以我們的經驗，以綠光雷射氣化術治療射精管開口阻塞，因為穿透力能有效控制，不易傷及括約肌，也不像傳統電刀對周遭組織有溫度傷害，得到了相當令人滿意的成果。（輔仁醫學期刊2011；9（3）：163-167）

Ejaculatory duct obstruction (EDO), although rare, is a surgically correctable cause of male infertility. Traditionally, standard treatment for managing EDO involves transurethral resection of the ejaculatory duct. We describe an infertile man with ejaculatory duct obstruction and the congenital absence of the vas deferens. The patient received photoselective vaporization of the ejaculatory duct (PVED). Motile sperm were noted 2 months later. In our case, PVED demonstrated an alternative and satisfactory treatment option. (FJJM 2011; 9 (3) : 163-167)
