摘要

本研究旨在探究護理人員在經歷照顧末期病人時承受的個人內在與專業困頓，藉以反思護理專業角色的成長與自我超越。為求能深入瞭解護理人員的主體性經驗，採用經驗性團體深度訪談的方式收集資料，成員來自台北某教學醫院安寧病房護理人員，計有15位，共舉辦8次的團體對話。以詮釋現象學方式進行資料歸納與分析，發現護理人員在照顧末期病人時浮現面對個人內在情感與專業困頓，包括內心糾葛、遺憾的力量、學習面對死亡；並繼續以專業與個人角色的相互滋養、生死兩相安二方面討論安寧療護護理專業的實踐與成長，期能透過關懷他人、認識自己，增強專業自我的敏銳覺察力，深度地瞭解病人和家屬的需求，提供更精緻的照護。

The aim of this study was to explore experiences caring for end-of-life patients with cancer, with a focus on the struggling between the inner and professional selves, in order to describe the psychological impact and self-transcendence as related to nursing professionals. Eight small group interviews were conducted with a total of 15 nurses from a hospice unit of a medical center, who had experience caring for end-of-life cancer patients. Group interviews were transcribed for analysis. Data were analyzed using the interpretative phenomenological method, revealing the following three intrapersonal and professional dilemmas: self-conflict, the power of resentment, encountering death. On the basis of the findings, the authors addressed the transactions between the professional self and personal self, and providing palliative care for the living and the dead. This study will lead to a new understanding of professional self and help nurses develop reflected care that promotes an empathetic understanding of clients' needs.
