摘要

脊髓損傷為人生遭遇之重大災難事件，損傷後患者面臨身體殘障、疾病帶來心理衝擊與失業之社會問題，憂鬱也成為損傷者常見之心理問題。近年來憂鬱造成自殺事件層出不窮，高自殺率也相對增加社會與醫療的成本。本研究目的為探討脊髓損傷者憂鬱程度及其相關因素，以橫斷性研究設計，採立意取樣以台中縣、市脊髓損傷者為研究對象共選取合乎條件個案共60位；研究工具為自擬問卷，內容包括基本屬性、疾病特性、巴氏量表、貝克憂鬱量表及社會支持，採面對面訪談方式進行資料收集，並使用SPSS 10.0套裝軟體進行統計分析。結果顯示脊髓損傷患者憂鬱平均得分11.97分，屬於輕度憂鬱48.3%；性別、加入協會與否與憂鬱程度呈顯著差異，社會支持與憂鬱呈負相關；經邏輯式迴歸分析發現「性別」與「社會支持」為預測脊髓損傷者憂鬱程度之重要影響因素；根據本研究結果可提供護理人員對脊髓損傷者憂鬱相關因素之了解，同時提供未來照護之參考。

Spinal cord injury is one of the most disastrous injuries a person may experience. Following such injury, patients must confront challenges including disabilities, psychological impacts and unemployment. Depression is a common problem amongst patients with spinal cord injuries. Depression also results in a high rate of suicides as well as increases social and medical costs. The purpose of this study was to explore the factors related to depression in patients with spinal cord injuries. The study employed a cross-sectional research design and purposive sampling was used. Sixty patients with spinal cord injury were recruited from among members of the Spinal Injury Association (SIA) in Taichung County and Taichung City, and data were collected using face to face, structured interviews. The questionnaire consisted of questions on demographics, illness characteristics, social support, Barthel Index and Beck Depression Inventory. Data were analyzed by SPSS Wins 10.0. The result demonstrates that 48.3% of patients with spinal cord injury suffered from depression, with a mean score of 11.97. Significant differences were found in terms of gender, participation in the SIA and degree of depression. Social support was positively correlated with depression. Logistic regression showed that gender and social support can help predict depression. Study findings may provide useful information to nurses about depression in patients with spinal cord injuries in order to provide quality healthcare.
