摘要


本文敘述照護一位過敏性紫斑症學齡前兒童的護理經驗,於2008年11月11日至11月20日期間,藉由生理、心理、社會文化、發展性、靈性五大層面進行評估,經由直接照護、身體評估、會談方式收集和評估相關資料,確立個案及其主要照顧者有體溫過高、疼痛、恐懼及照顧者角色緊張等四項護理問題。於治療中運用個案喜好之事物以緩解個案對住院及治療的恐懼感,應用同理心、主動關懷及探視個案與照顧者,適時協助及心理支持,提供資訊,使照顧者能面對疾病照護的問題,學會照護技巧,進而使個案重獲健康。

This case report describes the nursing care experiences of a 5 years-old child diagnosed with Henoch-Schonlein Purpura. Basic appraisal addressed five-aspects of care: physiological, psychological, social-cultural, developmental, and spiritual. Information was collected through interview, physical assessment and direct patient care. Patient problems were fever, pain, fear, and caregiver burden. Patient's favorite objects and play therapy were used to reduce fear toward hospitalization and treatments. Nursing interventions included caring and empathic approach to patient and the caregiver, timely assistance, and psychological support. Patient teaching and information provided to empower their self-care abilities contributed to the patient's recovery.
