壓力荷爾蒙對代謝適能及體適能影響之研究

A Study of the Relationship among Stress-Related Hormone, Metabolic and Physical Fitness

摘要


代謝症候群是心血管疾病許多危險因素聚集的現象,近年來許多學者研究發現肥胖、久坐工作生活型態、飲食、基因遺傳、環境因素等可能是導致此症候群的原因,已知壓力會對人產生綜合性的影響,層面擴及身體、心理、情緒及行為。因此本研究以台北市政府一級首長及一般職員為對象,就代謝適能及體適能作檢測,以了解壓力荷爾蒙對身體代謝及體適能產生的影響。本研究代謝適能檢測的項目包括:葡萄糖耐受度試驗、血清胰島素試驗、血脂肪(三酸甘油酯與膽固醇)、血清腎上腺皮質醇(cortisol)及血壓檢測;體適能檢測包括有:身體組成(BMI,體脂比、腰圍)、三分鐘登階、屈膝仰臥起坐及坐姿體前彎。研究結果發現,在代謝適能方面,北市首長在葡萄糖耐受度、血液三酸甘油酯與膽固醇、及血壓檢測結果,與一般職員並無顯著差異,但胰島素敏感性卻明顯地較一般職員差,有胰島素阻抗現象;體適能檢測結果,市府首長與員工在身體組成、三分鐘登階、屈膝仰臥起坐及坐姿體前彎均無顯著差異。結果顯示,在代謝適能及體適能尚未退化前,胰島素敏感性可為早期代謝性退化指標,而壓力性荷爾蒙-腎上腺皮質醇,則可能為影響胰島素敏感性下降的重要因素之一。

Metaboilc syndrome is characterized by a group of metabolic risk factors to cause cardiovascular disease. As known that abdominal obesity, low activity level, sedentary lifestyle, genetic factors, and environmental conditions contribute significantly to the risk of developing the metabolic syndrome. Stress includes not only the body's response to physical and psychological demands, but mental, emotional, and behavioral responses as well. The purpose of the study was to determine the relationship among stress-related hormone, metabolic and physical fitness. Officers and employees who works with Taipei city government were been chosen as participants. In the present study, the following metabolic fitness tests, including oral gluose tolerance test (OGTT), insulin level, triglycerides, cholesterol level, serum cortisol, and blood pressure level had been examined in all subjects. Body composition, cardiorespiratory fitness, muscle strength, and body's flexibility were evaluated as physical fitness in the study. The research showed that serum cortisol was a stress-related indicator. In the present study there was no significant difference between officers and employees in metabolic fitness, except insulin sensitivity. Meanwhile, there was no significant different in physical fitness either. But, the insulin response during OGTT was significantly higher in the officers' group. We suggested that stress-related hormone cortisol affect the metabolic fitness in the aspect of insulin response, and officers in Taipei city government should increase the capacity of physical activity to improve insulin resistance and reduce stressful cortisol to prevent metabolic syndrome.
