髕骨軸位攝影之X 光照射角度之評估

The Assessment of X-ray Tube Angle for Axial View of Patella

摘要


一般X 光攝影檢查的髕骨(patella)軸位攝影(axial view)主要是用來診斷退化性膝關節炎(Osteoarthritis knee ; OA knee),而在過去此檢查放射師操作X 光機及病人擺位過程繁複費時,使放射師及病患都很辛苦。因此若能研究改良傳統式的輔助墊,讓使用改良式輔助墊時放射師可以不需要再調整X 光機的角度,直接以0°拍攝,而同時病人擺位等待的時間也可縮短,藉此可簡化擺位過程。本研究分2 組,對照組(傳統式輔助墊)及實驗組(改良式輔助墊),每組取樣150 人,2 組分別計算時間,並加以統計平均,而2 組在計算後,實驗組的平均時間確實比對照組減少。本研究顯示運用改良式輔助墊拍攝髕骨之軸位攝影可以減少此檢查的時間。

Osteoarthritis of knee joint is a common disease from middle to old age in adults. The axial view of patella or patella-femoral joint in the plain X-ray image is one of the important examination for evaluating the osteoarthritis of knee joint. However, the image of patella is very technique dependent. The conventional axial radiograph of patella is performed with the patient sitting on the examination table and feet on the support cushion. The patient's knee is put to be in flexion 45°angle, the center-ray aimed at the patella's lower edge, with the tube tilting 13° cephalad. Consequently, the whole process is very time-consuming. The purpose of the study is to design a modified method with adjunct device to decrease the processing time of examination. The technologist just needs to aim at the patella's lower edge with a 0° vertical angle, with the angle the same as that of X ray tube of standing knee. In this prospective study, we divided patients into 2 groups: experimental group with modified positioning (n =150) and control group with conventional positioning (n= 150). We compared the processing time of examination in the 2 groups. The result showed there was less time for modified positioning as compared with that of conventional positioning (p < 0.05). In conclusion, the modified technique for X ray of patella with adjunct device shows the advantages of easy handling, time-saving and accuracy.
