高雄地區單一的第一型登革病毒血清與2001-2年第二型登革病毒及其流行之研究

摘要


由四型登革病毒(DEN-1, DEN-2, DEN-3, DEN-4)引起的流行目前仍是熱帶及亞熱帶地區極重要的公共衛生之問題。台灣地區登革熱流行主要在南部,自從1987-1988年高雄地區第一型登革病毒大流行之後,每隔三至五年即有一次較大的流行,2001-2002年高雄及屏東地區的第二型登革病毒大流行更是十多年來最大的一次流行。 根據流行病學上的觀察,高雄地區在2001到2002年間,雖有零星的第三型病毒之病例,但是並沒有造成第三型病毒的大流行,這與第二型登革病毒之大流行形成極大的對比。因此我們第一個假設為高雄地區過去有數次第一型登革病毒流行的紀錄(例如1987-1988, 1991),當地居民體內的對抗第一型登革病毒之抗體對第三型的病毒可能有相當的的保護效果,所以蚊子體內的第三型病毒無法在人體中大量繁殖,造成大流行。另一假設為2002年的流行可能與抗體促進效應有關,高雄地區的居民體內的對抗第一型登革病毒之抗體,會藉由Fc 受體促進第二型病毒的感染,使其在人體中大量繁殖,導致2002如此嚴重的流行 本研究的第一個目標是探討高雄地區2001-2年在人體或蚊子內之第二型及第三型登革之病毒基因序列及基因型。第二個目標是探討高雄地區單一的第一型登革病毒血清(monotypical DEN-1)對該區2001-2年第二型登革病毒的影響,是否有抗體促進效應,以及對第三型登革病毒之影響,是否有部分中和的效果。 我們首先分析2001-2002年疫情期間於高雄地區所捕捉的蚊子,發現在蚊子體內分別帶有第二型及第三型的登革病毒,進行序列分析後發現蚊子體內的第三型病毒,與台南1998所流行的第三型病毒為同一基因型(genotype),在部分E (envelope) 基因的序列非常相似;這顯示到2001-2年期間台南1998年所流行第三型登革病毒仍存在於蚊子體內,但可能在人類中傳播的效果不好,所以沒有引起流行。第二型病毒的序列分析顯示不論在人體或蚊子中都十分類似,2001-2病毒株在E基因沒有太大的改變,僅有一個胺基酸變化,位於第46個位置。 第二部分的研究我們由2002年6月大流行之前,在高雄三個地區(三民區、小港區、前金區)所收集的血清中,利用PRNT(plaque reduction neutralization test)找到25個monotypic DEN-1的血清,進一步探討這些血清對1998年的DEN-3及2001-2002年的DEN-2病毒之影響。 針對第三型病毒的部分,由PRNT的結果顯示,這些血清對於1998年的DEN-3病毒株與H-87相比,的確有較高的中和效價(p <0.05),這支持我們的第一個假說。對於DEN-2的抗體促進效應的部分,以分離健康者的周邊血液單核球細胞為感染的細胞,病毒的複製曲線在感染後第五天達到高峰,分析23個monotypic DEN-1血清對2001年DEN-2病毒株的影響發現,大多在稀釋100倍有較高的增強作用可讓病毒量增加1.7倍到6.8倍,以增加1.5為臨界值,18個(78%)血清具有抗體促進效應,增強2001年DEN-2病毒之複製,這支持我們的第二個假設。 本研究之整體目標是探討2001-2002年高雄屏東地區DEN-2大流行是否有一些生物學上的理由,特別是當地居民過去曾感染登革病毒之血清學情況(status)對這次流行之DEN-2病毒株之影響,以及該區DEN-3病毒株之影響,實驗結果支持我們的假說,顯示病毒-宿主交互作用對流行的重要性。雖然仍有釵h環境及人為等諸多因素影響是否造成大流行,我們的研究提供了一個可能的生物學上的解釋。未來衛生單位應可藉由類似的實驗方式,分析蚊子中病毒株的流行潛力,及早做出防範措施,減少疫情的嚴重度。

The epidemics of the four serotypes of dengue viruses(DEN-1, DEN-2, DEN-3, and DEN-4)continue to be the major public health problem in the tropical and subtropical area. After the DEN-1 outbreak in Kaohsiung in 1987-1988, the epidemic of dengue in Taiwan was characterized by a major outbreak every 3 to 5 years. In 2001-2002, the largest DEN-2 outbreak occurred in Kaohsiung. We first investigated the serotypes of dengue viruses present in the mosquitoes captured in Kaohsiung during 2001-2002, and both DEN-2 and DEN-3 viruses were detected. By using the approach of RT-PCR and direct sequencing, we found that the DEN-2 virus in mosquitoes was similar to the DEN-2 virus in patients during 2001-2002, and that the DEN-3 virus in mosquitoes was similar to the 1998 DEN-3 virus in Tainan. Based on the epidemiology of dengue in Kaohsiung, (predominantly DEN-1 outbreaks before 2001-2002, and about 90% secondary infection for the cases in 2001-2002), we hypothesized that monotypic DEN-1 sera are present at significant frequency in the population. Moreover, these monotypic DEN-1 sera can partially neutralize DEN-3 viruses circulating locally, and can have antibody-dependent enhancement(ADE)effect on DEN-2 virus of the 2001-2002 outbreak. By using the plaque-reduction neutralization test(PRNT), we identified 25 monotypic DEN-1 sera from three distinct in the Kaohsiung city. We then tested the effect of these monotypic DEN-1 sera on the DEN-3 virus of 1998 Tainan outbreak and the DEN-2 virus of the 2001-2002 outbreak in Kaohsiung. Results of PRNT indicated that these monotypic DEN-1 sera neutralized the DEN-3 viruses(360 and 368 strain)of the 1998 outbreak in Tainan more effectively than the prototypic DEN-3 strain, H-87 (p < 0.05). In the infectivity assay of peripheral blood mononuclear cell(PBMC), these monotypic DEN-1 sera showed ADE effect on the DEN-2 virus of the 2001-2002 outbreak at 1:100 dilution. The enhancement ranged from 1.7 fold to 6.8 fold. Overall, 18(78﹪)out of 23 sera tested had more than 1.5 fold of enhancement. Taken together, these findings support our hypothesis that monotypic DEN-1 sera can partially neutralize the DEN-3 viruses circulating locally and have ADE effect on DEN-2 viruses of the 2001-2002 outbreak. The overall objective of this study is to investigate whether these are biological reasons that may account for the largest DEN-2 outbreak in Kaohsiung. Our findings support our hypothesis and indicated the serological status of a population may determine whether a dengue virus strain can cause an outbreak in that population. Although several other factors such environmental and infrastructural factor may affect a dengue outbreak, our study provide a evidence that the dengue immune status of the host population may have influence on whether a particular virus strain can cause an outbreak.
