探討系統性護理指導對住院病童主要照顧者胸腔叩擊及姿位引流成效

摘要


本研究的目的在探討因呼吸道感染需要執行胸腔叩擊及姿位引流住院病童之主要照顧者,於接受系統性指導後的認知及技能之學習成效,採類實驗研究設計法前、後測量設計,以立意取樣方式選取中部某醫學中心兒科病房病童主要照顧者為對象。共收集200位病童的主要照顧者,其中實驗組100位接受系統性護理指導,由研究者根據文獻查證及多年之臨床經驗,協同小兒科醫護團隊,制定光碟護理指導容,拍攝成光碟學習系統,提供病童主要照顧者相關知識、技能之視訊學習方式,進行情境說明、知識教導及技術操作;對照組100位接受常規護理指導。於護理指導前、後利用「胸腔叩擊及姿位引流知識問卷」、「胸腔叩擊及姿位引流技能之評估量表」及「護理指導滿意度調查表」評估成效。結果顯示:實驗組於接受系統性護理指導後知識及技能實驗組進步的分數高於對照組,達統計顯著差異。滿意度包括態度、教材、指導時間及整體滿意度,實驗組滿意度皆高於對照組,達統計顯著差異。故經由光碟學習之系統性護理指導介入,對病童主要照顧者在胸腔扣擊及姿位引流之知識、技能及滿意度效果較常規護理指導好,應可做為臨床兒科護理人員提供護理指導之參考。

The purpose of this study is to investigate the result of nursing instructions on knowledge and skill of the respiratory infections, to primary caregivers who need to administer chest percussion and postural drainage on hospitalized children. A quasi-experimental study was designed and samples of participates had enrolled purposively in this study from a pediatric ward of medical center in middle Taiwan. A total of 200 primary caregivers of children were selected and divided into two groups. The 100 in the experimental group received systematic nursing instructions from the researchers who according to literature reviews and the clinical experiences of many years, collaborated with pediatric health care team to develop the CD education materials in that learning system was filmed into the CD-ROM to provide the children’s primary caregivers related knowledge and skills. Another 100 participates in the control group received conventional nursing instructions. The effects were evaluated using the ‘chest percussion and postural drainage knowledge questionnaire’, ‘chest percussion and postural drainage skills assessment scale’ and ‘satisfaction survey questionnaire’. The results show that after receiving systematic nursing instructions the experimental group gains higher scores significantly than the control group in knowledge and skills. The satisfaction Questionnaire includes attitudes, teaching materials, instruction time and overall satisfaction, in that experimental group had significantly higher scores than control group. Therefore, through the aid of CD learning on systematic nursing instructions, children’s primary caregivers show better results in knowledge, skills and the satisfaction of the chest percussion and postural drainage than conventional care. This should be able to serve as a frame of reference in nursing instructions for clinical pediatric nurses.
