巴金森氏症患者之圖形叫名探討

摘要


摘 要 研究目的:當額葉-皮質下迴路傳導失調時,除了會產生動作障礙外,人類動作/ 動作表徵功能是否也會有缺損,以至影響圖形叫名之動詞/ 動作相關字詞運作之表現?透過使用國語或台語為母語的巴金森氏症患者和正常人在圖形叫名作業的表現,來對巴金森氏症患者在動作相關字詞表現的缺損與否做一檢視與了解。並同時探討可能影響圖形叫名表現的因素。 研究方法:分為巴金森氏症以及正常人之兩組受試者,人數各為25人,並在性別以及教育程度變項予以控制。每位受試者都需接受相關的神經心理測驗以及情緒量表等篩檢工具的施測,再施以圖形叫名作業和視覺命名分測驗。使用混合因子共變數分析、單因子共變數分析,與皮氏相關法來進行研究假設的考驗。 研究結果:假設驗證方面顯示(1)相較於一般人,巴金森氏症患者在動作類別的字詞命名並沒有顯著比物體類別的字詞命名差。(2)巴金森氏症患者在視覺命名分測驗的表現並沒有顯著比一般人差。危險因子方面顯示(1)性別變項在視覺命名分測驗的表現產生顯著的影響,但在圖形叫名作業時並沒有發現。(2)年齡以及教育程度變項,與圖形叫名作業的表現都有顯著相關,而視覺命名分測驗的表現並未與年齡變項的相關達到顯著,但與教育程度有顯著相關。 結論:研究結果暗示額葉-皮質下的迴路和圖形叫名之動詞/ 動作相關字詞的運作可能沒有存在相對應的關係,但此仍有待未來的相關研究透過神經影像的資料來探討與釐清。

Abstract Purpose:When the dysfunction of Frontal-Subcortical circuits occurs, except the impairment in movement, would the function of human actions/ action representation be affected and subsequently influence the performance in the verbs/ actions related words processing of picture naming? By comparing the performance of Picture Naming Test between Parkinson’s disease patients and normal controls who use Mandarin and Taiwanese as their first languages respectively, the performance and related factors on the processing of verbs/ actions related words picture naming of patients with Parkinson’s disease were investigated. Method:A total of 25 participants with Parkinson’s disease and 25 healthy controls controlled by gender and educational level were recruited. Every participant needs to take related neuropsycholgical tests, Beck Depression Inventory-Second Edition, the Picture Naming Test and the Benton Naming Test. Mixed factors analysis of covariance (ANCOVA), analysis of covariance (ANCOVA) and Pearson correlation were used for testing the hypotheses. Results:The results showed that (1) comparing with normal controls, there was no significant difference between performance of action naming and object naming on Picture Naming Test in Parkinson’s disease patients. (2) there was no significant difference between Parkinson’s disease patients and healthy controls on Benton Naming Test. Regarding the risk factors, (1) the gender variable had significant influence on the performance of Benton Naming Test, but not on Picture Naming Test. (2) age and educational level were significantly related to the performance of Picture Naming Test and Benton Naming Test. Although significant correlation was found between the performance of Benton Naming Test and educational level, the correlation between the performance of Benton Naming Test and age was not significant. Conclusion:The results did not support the relation between Frontal-Subcortical circuits and verbs/ actions related words processing of picture naming. However, it should be further investigated by neuroimage research in the future.
