台北市老人失能評估社工員之自我角色與功能定位之探討

摘要

台北市老人失能評估在社會局主導約有10年歷史，此段期間，政策面的轉變與社工員角色間產生許多互動與變化。本研究目的旨在瞭解居家服務政策規範的轉變，與其衍生之正反面的效應，同時瞭解社工員在其間的自我角色與功能之詮釋，以及過程中遭遇到的相關議題。
本研究採深度訪談方式，訪問8位台北市老人中心社工員，以及2位社會局居家服務承辦人員。研究發現政策面對於居家服務的規定從單一的指標轉向多元指標、服務內容也從粗略的大項目轉而對服務安排細節的要求、專業自主的空間也因為覆審制度的設置而產生消長變化；社工員自我角色與功能從單純的失能評估者，歷經居家服務個案管理者，再擴及為失能個案管理者，從評估單一需求的任務發展到解決案主多元需求之個案管理。
研究建議分為社會工作實務面、制度面，與衛政體系三部份。社會工作實務面部份建議社工員自我充權、善用社工專業技巧於工作團隊、提升社工專業知能與再教育、增強社工員專業自信與以案主需求為導向的思考；制度面則建議規範清楚的職掌與任務、針對居家服務單位訂定具體的獎懲規範、協助建構與擴充社區服務資源、充足的專業人力與促進評估單位與服務單位間的交流；對衛政體系的建議則為確立失能評估的本質與內涵、與老人中心個案管理的議題須定調、彈性多元的評估機制、長照中心專業人力配置比例、參與居家服務的會報與評鑑工作、行政程序再簡化以提升服務品質、社政經驗的傳承、辦理多樣化且多樣化且實務色彩濃厚的教育訓練課程、增進各區督導的專業與評估能力，提升評估一致性。

The assessment of the disabled elderly has been conducted by the Bureau of Social Welfare in Taipei for approximately ten years. During this period, the policy of the home care service has been amended, which also affected the functions of social workers. The purpose of this study is to understand the policy changes of the home care service and the impacts. In the meanwhile, this study also tried to understand how social workers interpreted their own professional roles as well as the related issues which social workers encountered.
This study used in-depth interviews for eight front-line social workers and two administrative staffs. The major findings of this study were：（1）the autonomy of social workers had been changed due to the case re-examination system which was implemented by administrators.（2）the professional role of social workers had been changed from pure evaluators to home service case managers, and then has been expanded to the case managers of the disabled elderly. Finally, the policy recommendations of this study were presented in terms of social work practice, social welfare system and the health system.
