勞工健康檢查制度現況與問題之研究--台北市醫療機構個案分析

摘要


本研究以管制理論作為理論基礎,來探討目前勞工體檢的問題,並提出改善策略來提昇勞工健檢的品質及相關制度。 本研究有幾項重要發現:首先針對非指定醫療機構加入健檢市場,醫療健檢部門、勞工及衛生主管機關均表反對意見。理由如下所述,除擔心體檢品質下降,也怕削價競爭的情形日趨嚴重,另外也質疑規模較小之醫療機構之檢驗水準。同時全面開放體檢市場後,主管機關很 難進行管理,勞工更無從選擇優良的指定醫療機構。其次發現目前勞工體檢市場低價競爭及推銷自費項目情形嚴重,讓事業單位無法選擇,感到十分困擾。所以公司一致希望能制定合理及兼顧品質的收費標準,讓公司選擇體檢醫療機構時有所依循。最後,綜合研究發現,提出政策建議以為未來相關法規修正之參考,本研究提出以下建議:(1)建議主管機關制訂合理體檢收費標準。(2)建議政府建立完善職業健康服務制度。(3)政府主管機關應加強稽查管理指定醫療機構。(4)建議建立職業健康資料庫與管理系統。(5)從嚴審核指定醫療機構之申請。(6)建立指定醫療機構醫事人員管理系統。(7)加強宣導勞工體檢的重要性。

The purpose of this study, based on regulation theory, was to explore the labor physical examination, and then propose strategies to enhance the quality of the labor health examination and related systems. The findings were as followed: 1. Labors and people from the departments of medical health examination and the organizations of public health disagreed about the unassigned medical organizations joining in the health examination market. The reasons were the fear of the decrease of the quality of the labor health examination, the serious price war and the inspecting criterion of the smaller medical organizations. The authorities might have difficulty in management; meanwhile the labors might not pick medical organizations reasonably if there were an opening physical examination market. 2. The public institutions might not choose and felt perplexed because of the price war and sales promotion for privately items in present labor physical examination market. Therefore, people in the company expected the reasonable charge standard with great quality which people could rely on. 3. There were seven policy suggestions as followed: (1) To draw up the reasonable price of the labor physical examination. (2) To establish a complete professional health service system.(3) To inspect the assigned medical organizations.(4) To establish occupational health information bank and management systems.(5) To verify severely the applications of assigned medical organizations. (6) To establish personnel management systems of assigned medical organizations.(7) To enhance public awareness of the importance of labor physical examination.
