保守治療對肘部肌腱炎的效果

Result of Conservative Treatment for the Tendinitis of Elbow

摘要

自民國73年年初至民國75年年底，因肘部骨腱炎而接受保守治療的患者中，作者總共收集到資料齊全的91位患者，其中有2人因雙肘皆發生肌腱炎，另有2人，同一肘的內、外上髁皆具肌腱炎，因此所研究的對象共95例。以治療方式的不同分爲4組：第1組，接受局部注射、超音波治療，以及口服藥。第2組，僅接受局部注射及口服藥。第3組，接受超音波治療及口服藥。第4組，僅接受口服藥。
結果以第1組的治療效果最佳，完全痊癒的有57.5%，症狀明顯改善的有35.0%；第2組次之，完全痊癒的有67.9%，明顯改善的有17.9%；以第4組的效果最差，僅33.3%完全痊癒，屬於無效的患者，竟達66.7%。雖然局部注射的效果快速且經濟負擔較輕，但超音波治療的復發率較低。如果僅接受局部注射及口服藥，復發率是42.1%，但曾接受超音波治療的患者，其復發率僅21%~25%。

From the beginning of 1984 to the end of 1986, there were 91 patients who got the tendinitis of elbow and received conservative treatments. Among the 91 patients, there were 2 patients who got bilateral tendinitis of elbows and another 2 patients who got both of epicondylar tendinitis at the same elbow, so that all of the total series in this study included 95 cases of the tendinitis of elbow.
According to receiving different treatments, all of the cases were divided into 4 groups. Gr. Ⅰ: The 40 cases not only received local injection, but also received ultrasonic diathermy and medication. Gr. Ⅱ: The 28 cases only received local injection and medication. Gr. Ⅲ: In this group, 24 cases received ultrasonic diathermy and medication, Gr. Ⅳ: The 3 cases just took medication only. All patients were followed up by phone in April, 1987. After analizing covery. 17.9% of this group population got much improvement. The treatment result for cases in the 4th group were the worst, because only 33.3% of this group got complete symptom relief, and 66.7% didn't have any response to the treatment.
The group receiving local injection with the least treatment cost shows significantly good results in symptom relief among the groups. But it has higher recurrent rate than the group receiving ultrasonic diathermy.
