Using Minocycline Sclerosing Therapy for the Managenment of Chyle Leakage After Radical Neck Dissection for a Recurrent Oral Cancer Patient

乳糜液漏是頭頸部惡性腫瘤，進行頸部淋巴廓清手術後，造成傷口延遲癒合的原因之一。手術後的乳糜液漏，會造成傷口感染、電解質失衡、頸部皮膚壞死，嚴重時甚至會導致敗血症而造成死亡。在文獻中曾有許多方法被提出用來處理這類的問題，例如重新手術結紮、局部壓迫或藥物處理等。本病例為一左下齒齦癌經腫瘤切除手術後，發現左側頸部淋巴轉移，而進行第二階段之左頸部淋巴廓清手術，因手術過程傷及胸管，造成術後嚴重之乳糜液漏，傷口延遲癒合，最後採用局部注射硬化劑-(minocycline) 的硬化療法(sclerotherapy)，而使傷口情況獲得控制、癒合之病例報告。

The after effect and complications from a prolonged chyle leakage after radical neck dissection (RND)often cause wound infection or delayed healing, extreme electrolyte imbalance, skin flap necrosis, or even death. Diverse methods have been reported to treat this complication, ranging from conservative management to surgery. In this case report, we described an oral cancer patient who had a thoracic duct tear during RND, which caused a severe post -operative chyle leakage, and was cured by minocycline sclerotherapy.
