非侵入式糖化血色素光譜檢測於糖尿病應用可行性研究

摘要


糖化血色素 (Glycated Hemoglobin, HbA1c)是糖尿病的診斷與監測的重要標誌。而在血色素與葡萄糖產生糖基化過程所造成的血色素改變成為光譜檢驗糖化血色素濃度的重要指標。在本研究中,我們分析了利用光譜量測技術對糖化血色素濃度檢測的可行性,而光譜檢測技術不僅是一個敏感度高的檢測方式,同時也具有快速檢測、低成本的檢測系統,並配合漫反射式光譜檢測系統以達到非侵入式糖化血色素光譜檢測系統。 在實驗中,為了確立不同濃度的糖化血色素可藉由光譜檢測,因此進行了血色素糖基化實驗觀察過程中血色素光譜的變化情況,利用帶氧血色素、缺氧血色素與高鐵血色素的光譜特徵波峰進行光譜比值,由結果我們確立不同濃度的糖化血色素能以光譜量測系統檢測,且於糖化血色素濃度為1%~12%時擁有98%的量測線性度。此外,我們還進行了漫反射光譜於糖化血色素的非侵入式檢測的可行性實驗,於仿體漫反射光譜之模擬量測結果中,糖化血色素為3%~12%的量測線性度也達97%;於未來將針對臨床量測部位進行該部位組織散射係數與黑色素的等誤差因子訊號干擾模擬以建立量測校正參數,並將此校正參數實際應用臨床量測以分析非侵入式糖化血色素檢測的可行性。

Glycated hemoglobin (HbA1c) is an important maker in diabetes diagnosis and disease status tracking. In this study, we demonstrate the advantage of using absorption spectroscopy technique for glycated hemoglobin (HbA1c) quantitation. By the ratio of selected absorption peaks, the concentration of HbA1c can be easily determined. Our experimental result shows that absorption spectroscopy is not only a sensitive method in HbA1c detection, but also a high potential technique in developing a noninvasive, rapid and cost effective diabetes monitoring device with. In our experiments, in order to establish the HbA1c concentration can be detected by spectral, we used the feature peaks of oxy-Hb, deoxy-Hb and metHb to calculate the peak ratio. As the results shown, we demonstrated that different HbA1c levels can be detected by spectral measurement system, and the calibration curve had 98% linearity in 1% to 12% HbA1c levels. In addition, the calibration curve had 97% linearity in 3% to 12% HbA1c levels of phantom spectrum by using the diffuse reflectance spectroscopy in noninvasive measurement experiments.
