營養師介入對居家照護管灌個案營養狀況的影響

摘要

針對羅東博愛醫院居家管灌照護個案，於營養師介入後，評估個案的營養狀況是否改善，其體重、體脂肪與生化值的變化以及家屬是否從營養師的營養照護中得到幫助。研究自民國93年6月25日至95年1月25日，分為二階段: 第一階段24週的營養介入與第二階段第76週的問卷調查。共22位個案完成研究，平均年齡76.2歲。共分為在第一階段持續24週的營養介入後，體位測量僅在上臂圍與上臂肌肉環圍有顯著的增加，體重則呈現上升之趨勢。在熱量、蛋白質、脂肪的攝取上有顯著增加。營養評估採用MNA與SGA，但無顯著差異。第二階段的問卷調查結果，對營養師訪視的各項滿意度達79-90％，對於營養所做的各項飲食協助有63-89％有得到協助。在營養師諮詢費則傾向於由政府補助。結論:對於居家管灌個案，營養支持是重要。因此，營養師的介入照顧是必要的。

To analysis the effects of nutrition support of home care enteral feeding patients from 2004 to 2005 at the area of Ilan Hsien, Taiwan. Participants were 22 home care enteral feeding patients who were discharged from Lotong Pohai Hospital and the mean age was 76.2 years old. To conduct a nutritional intervention program to these patients 6 months and analysis the effect. Anthropomteric data only have MAC、MAMC were improved significantly. Nutrition screen score were not improved significantly. There were significantly increased in calorie, protein and fat intake during dietitian intervened period. 49-90% of care givers were satisfied with dietitian severvice, 63-89% of care givers got benefit from dietary adjusted by dietitian. Nutrition support is a important medical procedure for home care enteral feeding patients.
