本研究目的在提供一套有效又不具侵入性的重建外觀之「心靈小 棧」介入措施,並探討心靈小棧對改善乳癌手術婦女身體心像及情緒狀態之成效。本研究採類實驗性研究設計( Quasi-experimental design ),以立意取樣的方式於南部某區域教學醫院兩院區之外科病房,選取接受改良式根治性乳房切除術之乳癌婦女為研究對象,以設有心靈小棧之院區作為實驗組,沒有設心靈小棧之院區作為對照組,有效樣本共32人,包括實驗組20人,對照組12人。實驗組除一般乳房切除術之護理外,並給予兩次心靈小棧介入措施,而對照組僅接受一般乳房切除術之護理。主要研究工具包括基本資料表、身體心像量表、情緒狀態量表及視覺類比量表。實驗組於手術前且未實施心靈小棧措施前、出院當天及術後兩個月各測量一次,對照組測量時間與實驗組一致,實驗組並於後測同時填寫心靈小棧評值表。研究結果發現接受心靈小棧之實驗組較對照組能顯著地改善身體心像 ( t = 6.10,p< .0001 ) 及情緒狀態 ( t = 5.64,p< .0001 )。在心靈小棧的評值方面,實驗組患者均對心靈小棧護理指導感到滿意。依研究結果建議醫院未來能成立類似心靈小棧的場所,以協助即將接受乳房切除術的乳癌婦女獲得義乳資訊,提升對乳癌手術婦女的照顧品質。
The purpose of this study was to provide a Mood Pacifying Service consisting of a set of methods as a non-intervention appearance reconstruction, and explore the effects of mood pacifying services on body image and emotional status of breast cancer women with modified radical mastectomy (MRM). Patients who had undergone MRM were recruited from two branches of a regional teaching hospital in southern Taiwan. Those in the mood pacifying service’ branch were assigned to the experimental group, while those not, were assigned to the control group. A quasi-experimental design was used in which thirty-two patients including twenty experimental subjects and twelve control subjects completed the study. The experimental group received traditional routine care and mood pacifying services twice, while the control group received only the traditional routine care. The instruments utilized in the study were a Demographic Questionnaire, a Body Image Scale, an Emotion Status Scale, and a Visual Analog Scale. The experimental group had filled out the Evaluation Form of Mood Pacifying Services in the post-test period. The results indicated that the experimental group had significant improvements on the body image ( t = 6.10,p< .0001 ) and emotional status scales ( t = 5.64,p< .0001 ) than the control group, as evaluated at the end of two months. All the patients in the experimental group were satisfied with the mood pacifying services. Results from this suggest that similar mood pacifying services should be set up in the hospital in the future. They can help breast cancer women with MRM get external breast prosthesis, information, as well as promote the quality care of women with breast cancer surgery.
