以情感理論模式探討影響急診醫療團隊工作態度之要素

摘要


研究背景與動機 近年來醫院評鑑越來越重視「醫療團隊」,而臨床工作的忙碌容易讓團隊成員忽略彼此是一個團隊,並且缺乏了「溝通」,殊不知溝通是團隊績效彰顯的基石,因此研究醫療團隊溝通之議題刻不容緩。此外,在醫療體系中員工長期存在高度的情緒勞動,會導致自我異化,有可能間接影響工作態度(Hochschild, 1983)。故本研究將團隊氣氛、團隊溝通與情緒勞動及工作態度導入情感理論模式,進一步去探討人格特質是否會影響到醫療團隊的整體運作。 研究目的 本研究導入情感事件理論來探討急診醫療團隊之團隊氣氛對團隊的工作態度的直接影響,以及團隊氣氛透過團隊溝通、情緒勞動對團隊工作態度的間接影響;並進一步探討團隊成員的人格特質在團隊溝通與情緒勞動間是否具有干擾效果。 研究方法 本研究採橫斷面研究以問卷調查法進行研究設計,並針對八家醫療機構之急診醫護人員為對象,總共發放 435 份問卷,有效問卷 213 份,有效回收 49%。並以 AMOS 21.0做為結構方程模式驗證以及 SPSS 18.0 進行干擾效果的迴歸分析。 研究結果 研究結果發現急診團隊氣氛會正向影響團隊溝通及工作態度、團隊溝通會正向影響情緒勞動、情緒勞動會正向影響工作態度,同時團隊氣氛亦會透過團隊溝通、情緒勞動對工作態度有正向影響。另外,急診醫療團隊員工人格特質會調節團隊溝通與情緒勞動之間的關係,至於內控人格特質對會進一步強化團隊溝通與情緒勞動之間的關係。 研究建議 建構良好的支援體系,提升團隊氣氛並且落實團隊溝通,以降低急診團隊情緒勞動負擔。本研究是由急診團隊成員進行填寫團隊內部之氣氛、溝通、情緒與人格特質、工作態度,因僅探討至態度面,無法針對成員與主管之間進行配對,因此,建議未來研究可利用配對問卷來深入探討至急診團隊的工作績效。

Background In recent years, working with medical team is an important issue in hospital accreditation. Busy clinical works are easy to ignore the team work and lack of team communication. Team communication is the basic of the team performance. Therefore, it is urgent to investigate the issue of medical team communication. In addition, employees worked over a long period of time in highly emotional labor of the health care system will lead to self-alienation, it may indirectly affect the job attitude. Purpose This study applies affective events theory (AET) to explore the direct impact of Emergency team climate on the job attitude, the indirect impact of team communication and emotional labor on job attitude. Further this study finds the moderating effects of personality trait on team communication and emotional labor. Methods In this cross-sectional study, we invited eight hospitals including 435 individuals working in emergency room to fill out the questionnaire. Two hundred and thirteen individuals were performed the questionnaire. The statistical analyses in this study were performed by SPSS18.0 and AMOS 21.0 software. Results In this study, we observed: (1) Emergency team climate revealed remarkable positive influence on team communication and job attitude. (2) Emergency team communication revealed remarkable positive influence on emotional labor (3) Emergency emotional labor revealed remarkable positive influence on job attitude (4) Emergency team climate through team communication and emotional labor will affect job attitude. (5) personality trait moderated the relationship between team communication and emotional labor. In addition, the internal control personality will strengthen team communication and emotional labor relationship. Conclusions and Implications Constructing a good clinical support system enhanced the team climate, team communication, and reduced the emotional labor of burden in emergency team. This study only discusses team climate, team communication, emotional labor, personality traits and job attitude member, the relationship between members and supervisor still need to discuss. Therefore, it is suggested that matching questionnaire is valuable to study the emergency team job performance in the future.
