冠柱之材料性質與幾何條件對根管治療後牙齒於冠柱體穩定度的影響

摘要


在根管治療之後的牙齒,由於已失去了牙髓腔中神經、血管、淋巴等營養供應,因此根管治療後牙齒的復形為一重要課題。使用冠柱體為常用方法之一。本研究目的在使用有限元素法來分析植入不同材料性質、不同幾何條件之冠柱體對於根管治療後牙齒的穩定度的影響。為了模擬牙齒之真實情況,使用不同的接觸條件套用到模型之中,並做比較與分析結果探討。本研究使用有限元素模型前後置處理軟體Mentat 以及Marc 2003有限元素分析軟體進行分析,分別模擬咬合力50 N、以及唇側10 N微小傷害力之負載;分析結果顯示,使用樹脂材質比起金屬材質較沒有明顯之應力集中現象,並且可使應力較為平均分布。研究中並針對牙本質與冠柱間接觸特性做探討,假設牙本質與冠心之間有摩擦力時,根柱直徑增加對於冠柱體的位移呈現下降的趨勢。並且有助於減低根柱之應力以及牙本質之應力。假設牙本質與冠柱為一連續體時,愈寬的根柱直徑對於整體應力分布沒有顯著的影響。欲使用樹脂冠柱替代金屬冠柱,直徑需比金屬冠柱為大,才能達到金屬冠柱的效果。

The restoration of endondontically treated teeth is an important issue because the loss of nerve, blood vessal and lymph of the canal following the treatment. Currently, the post-core system is one of the commonly used restoration methods. The purpose of this study was to investigate the effects of post-core material property and geometry on the stability of the post-core system for endodontically treated teeth by using finite element analysis. In order to simulate the real situation, two different interface conditions (bonded and contact) were considered to investigate the effects of bonding and debonding in the post-core-dentin interfaces. Commercial finite element analysis softwares (Mentat 2003 and Marc 2003) were used. Loading conditions of a masticatory force and a horizontal load were analyzed. The results showed that the metal post and core caused higher von Mises stress than the composite resin did. On the other hand, for the composite resin post and core system, the stresses were more evenly distributed. When contact condition was assumed for the dentin/core interface, as the diameter of the post increased, the open-up displacement between dentin and core decreased. This can be beneficial in reducing the stresses in the post and dentin. When fully bonded condition was assumed for all the interfaces, it has no significant difference on the stress distribution. The open-up displacement was not affected by the length of post length when composite resin post-core system was used. When the composite resin post-core system was used to replace the metal post-core system, the diameter of the resin post is recommended to be wider than the metal post in order to obtain the equivalent result.
