醫院病患伙食自辦或委外決策因素之研究

摘要


從民國八十四年三月開始實施全民健保制度後,隨著人民對健康的重視程度、預防保健需求的提升,造成全民健保醫療費用日益增加。總額預算制度實施後,雖然能有效地控制健保醫療費用的增加,但醫療院所必須面對總額預算制度實施後所帶來的利潤壓縮之壓力,且還要準備新制醫院評鑑,以達到政府及民眾要求的醫療品質水準。醫院為突破經營困境,促使醫院重新審視及調整經營策略。 本研究以四個區域教學等級的醫院為研究對象,探討醫院病患伙食自辦或委外的決策因素,藉此作為醫院病患伙食決策的參考依據,希望能替住院病患帶來更好的服務及品質。 經由實地訪談及資料分析得到以下結果: 1.經營者注重品質的策略,是醫院病患伙食自辦之因素。 2.外包商配合度及管理能力不佳,是醫院病患伙食自辦之因素。 3.經營有獲利空間,是醫院病患伙食自辦之因素。 4.人員配合度高及穩定性高,是醫院病患伙食自辦之因素。 5.產品品質及管理易於掌控,是醫院病患伙食自辦之因素。 6.外包商的專業能力佳,是醫院病患伙食委外之因素。 7.品質良好及配合度高的外包商,是醫院病患伙食委外之因素。 8.高人力配置,是醫院病患伙食委外之因素。 9.高經營成本,是醫院病患伙食委外之因素。 10.專注於核心業務發展,是醫院病患伙食委外之因素。 11.高行政成本,是醫院病患伙食委外之因素。

The National Health Insurance (NHI) program in Taiwan was instituted in March 1995. Since then, along with people’s awareness to health issues and increasing needs for preventive medicine, the medical expenditure of NHI has increased. Although the increase has been effectively controlled after the adoption of a global budget system, the healthcare system was then under the pressure of profit squeeze. Moreover, hospitals have to prepare for New Hospital Accreditation in order to meet the quality and standard that the government and people expect. The management policies of hospitals have to be reviewed and adjusted in order to overcome the difficult situations. The subjects of this study are four regional teaching hospitals. This study discussed the reasons why hospitals choose to self-operation or outsourcing patient catering service. Findings of this study provide implications in the decision making of patient catering poilicy, so that better quality and service can be provided to inpatients. The following is the result from field interviews and survey data: 1.The reasons that the hospitals choose to self-operation patient catering service : concerns over the quality of outsourced catering service. 2.The reasons that the hospitals choose to self-operation patient catering service : poor cooperation and management of the contractor. 3.The reasons that the hospitals choose to self-operation patient catering service : profit margin. 4.The reasons that the hospitals choose to self-operation patient catering service : high cooperation. 5.The reasons that the hospitals choose to self-operation patient catering service : stability of hospital staff. 6.The reasons that the hospital outsourcing patient catering : good professional ability of contractors. 7.The reasons that the hospital outsourcing patient catering : contractors with high quality and cooperation. 8.The reasons that the hospital outsourcing patient catering : high cost of hospital man power. 9.The reasons that the hospital outsourcing patient catering : high operating costs. 10.The reasons that the hospital outsourcing patient catering : the intention to focus on core business. 11.The reasons that the hospital outsourcing patient catering include: high administrative costs.
