摘要


論文摘要 隨著醫療科技的發達和全民健保的實施,各項預防保健政策推廣,國人平均餘命不斷的提升,人口結構老化已成為不能避免之趨勢;再者,因雙薪家庭比例增加,家庭結構的改變,老人安養和照護問題帶動長照機構之需求日增,而老化導致身體各項功能退化,老人常有慢性疾病或是多項疾病纏身,視力聽力和認知變差,對藥物的需求和依賴比一般人更重,老人是用藥的高危險群,長照機構的老人用藥議題更需加以重視。 本研究是以長照機構的住民用藥安全為考量,以資訊功能為輔助,採以雲端技術建置一套住民用藥評估系統,透過系統功能對於住民之用藥依照藥品成份自動交叉比對、進行偵測、在重複用藥和藥物交互作用上面進行主動核對,再由專業藥師做最後之覆核,對於各住民之用藥提供更詳實和具體之建議。 此篇論文以系統建置做為主要研究方法,以雛型式生命週期法(prototyping life cycle)為系統開發方式,程式設計方面委外於資訊系統公司,藉由不斷的溝通和改進,並於系統施行後做問卷評估和主管訪談,以便能了解使用者滿意度以及尚待改進之處,提供開發者修改之參考,期望能持續並推廣至其他機構使用,可對長照機構內的住民用藥安全提供更嚴謹的把關。 關鍵字:長照機構、雲端技術、老人用藥、交互作用

Abstract The advances in medical technology, the implementation of national health insurance, and the promotion of healthcare policies raise Taiwanese life expectancy, and therefore make the population aging an inevitable tendency. Besides, the rising percentage of dual-earner households and the change in family structure cause the problem of elderly nursing and healthcare, and therefore stimulate the demand for long-term care institutions. Moreover, aging will lead to the degradation of organic functions. This is why the elderly often suffer from chronic diseases or other physical problems. The impairment of visual and hearing abilities and the deterioration of cognition forced the elderly to rely on medication much heavier than ordinary people. The elderly is a high-risk group of population taking medication. We must pay extra attention to the issue of elderly medication in long-term care institutions. This thesis primarily concerns the safety of elderly medication in a selected long-term care institution, and takes information technology as an auxiliary tool, namely establishing a system for medication evaluation through cloud technology. This system is able to automatically detect and cross-check the pharmaceutical composition of the elderly medication in the institution, and spontaneously check the repetition of prescription and the interaction between different drugs. Professional pharmacists will conduct a final review of the result produced by the system and then provide much more detailed and concrete suggestions on the elderly medication of the institution. System building is the primary research method of this thesis, and prototyping life cycle is the approach to build the system. The frequent communication with the IT company improves the quality of the outsourced programming. The analysis of questionnaire and the interviews with the director of the selected long-term care institution after the application of the system will clarify the degree of user’s satisfaction with the system and the problems yet to be solved. The result of the investigation provides the program designer with a reference for improving the system. The system for medication evaluation is expected to be promoted and used by other long-term care institutions, and thereby serves as a more rigorous safety control for elderly medication.
