隱藏性肥胖與代謝症候群之探討

摘要


背景:肥胖與健康是二十一世紀重要議題,身體質量指數(body mass index, BMI)更成為世界肥胖通用指標。但即使身體質量指數(BMI)正常,但會因體脂肪(body fat)過高,造成隱藏性肥胖(hidden obesity),這些人是代謝症候群(metabolic syndrome, MS)的高風險者,但卻常被忽略。本研究目的了解民眾隱藏性肥胖與代謝症候群之情況與關聯並找出其影響因素。 研究方法:以某國際連鎖健康管理機構2011年會員健檢資料為調查對象,採性別分層隨機抽樣選取1140個有效樣本。針對其健檢資料包括基本變項、生活型態進行描述性統計,包括個人基本變項與隱藏性肥胖、代謝症候群之分布情況,並以卡方檢定、邏輯斯迴歸等分析性統計,討論變項間關聯與差異。 研究結果:隱藏性肥胖平均盛行率為14.6%,女性(18.9%)顯於高於男性(8%);代謝症候群平均盛行率為14.9%,男性(21.2%)高於女性(8.7%)。代謝症候群與隱藏性肥胖呈現正相關。在控制基本變項與生活型態變項後女性、有代謝症候群、管理類工作、學歷高中以下者得到隱藏性肥胖的風險較高;技術職、國中以下學歷、有隱藏性肥胖、BMI值過重或肥胖的人得到代謝症候群的風險也較高。 結論:隱藏性肥胖是一個容易被忽略的健康問題,尤其是對BMI值偏瘦或標準的人。本研究結果發現女性得到隱藏性肥胖的機率高於男性,而男性則有較高的機率罹患代謝症候群。因此建議舉凡健康管理相關機構在執行健檢工作時,除了應將體脂肪率納入健康檢查得基本項目,並對隱藏性肥胖者加強如代謝症候群相關疾病監測。

Objective:Obesity is one of the twenty-first century important health issues, body mass index (BMI) became the world''s most common indicator of obesity now. Even someone’s BMI was normal, but its body fat (BF) is possible abnormal, and should result in the hidden fat problems which may be easily ignored and to be the high riskier of metabolic syndrome (MS). Therefore, this study tries to understand and find the important factors of the hidden obesity associated with metabolic syndrome. Methods:By systematic sampling 1140 valid samples from the 2010 data base of a health examination center. Research data mainly from two parts, one was their health survey data; subjects should complete their questionnaires before the health examination. And the questionnaire includes basic personal information, lifestyle and so on. The other one was the records of health examinations. The data will process by descriptive statistics to explore the distribution of basic personal variables, hidden obesity and metabolic syndrome. And then, by the chi-square test, to analyze relations and impact of personal Information variables, lifestyle items, hidden obesity stauts, and metabolic syndrome. After screening the related factors of metabolic syndrome, furthermore to discuss odd''s ratio (OR) and process the single factor logistic regression analysis in order to find out the risk factors of metabolic syndrome. Result:The hidden obesity total prevalence of this study was 27.4%, in female group(18.9%) was significantly higher than male(8.0%). And the metabolic syndrome total prevalence was 27.4%, male group(21.2%) was significantly higher than female(8.7%). Hidden obesity was associated with metabolic syndrome. After controlling other variables, female, manager, and whom with the high-school diploma had the higher prevalence rates of hidden obesity. Furthemore, the people who with hidden obesity, high BMI which obesity, worked at engineering, or only gratuated from junior high school, had higher prevalence rates of metabolic syndrome were higher them the thers. Conclusion:The hidden obesity problem is easily to be ignore, overlooked especially happened in slim female who got the standard BMI. We found women might of hidden obesity, and men got a higher probability of metabolic syndrome. Therefore, we suggest that health examination should be include measuring the rate of the factors of metabolic syndrome .
