以灰色預測模式推估臺灣地區皮膚科專科醫師人力供需與地理分佈研究

摘要


背景:醫師人力的供需是臺灣重要的衛生政策議題,由於各醫學專科之醫師人力供需失衡日趨嚴重,衛生署遂進行調整各專科醫師之訓練額度,其中皮膚科由歷年平均約35名額度,調降至2011年的18名與2012年的14名,大幅調降是否符合未來需求,實應有良好的皮膚科人力供需預測模式為基礎。因此本研究擬進行皮膚科醫師人力供需之推估,作為政策評估之依據。方法:本研究採用灰色預測模式,以衛生署醫療統計年報之皮膚科就診人次資料及醫師公會的醫師人力資料為研究材料,推估2011至2015年台灣地區皮膚科醫師人力供需,並評估其地理分佈情況。結果:2011至2015年皮膚科醫師於均呈現供過於求的現象,未來五年供需失衡比率由5%成長為8.5%。但在地理分布上,台灣368個鄉鎮市區中,有246個地區沒有皮膚科醫師提供服務,比率高達66.8%,可見皮膚科醫師人力存在嚴重的地理分布不均問題。結論:衛生署大幅降低皮膚科訓練容額,有助於減緩當前皮膚科醫師人力供過於求的問題;但皮膚科醫師人力地理分佈不均,都會區與鄉村區相差甚巨,嚴重影響民眾就醫的便利性。建議:政府於醫師人力供需的政策上,除了考量數量供給外,如調整醫師訓練容額,或許有助於減少皮膚科醫師人力供過於求問題,但於地理分布不均的問題上,也應有更積極政策,如由政府以經濟誘因或政策介入模式,以解決民眾就醫可近性之權益保障。

Background: The supply and demand of physician human resource is one of the most important health policies since the imbalanced problem of physician human resource is getting worst. Recently, the department of health in Taiwan has adjusted the number of physicians in all specialists. The number of dermatologists drops from 35 to 18 in 2011 and to 14 in 2012. However, it is necessary to investigate whether the adjustment can meet the patients’requirement. Method: Based on the number of patients reported in the department of health and medical association, this research adopts grey theory prediction model to estimate the supply and demand of dermatologists in Taiwan in 2011 to 2015. Moreover, the geographic distribution is also taken into account. Result: The dermatologists are surplus to requirement in 2011 to 2015, and the imbalanced ratio is raised from 5% to 8.5%. As for geographic distribution, 246 districts in Taiwan, where is consisted of 368 districts, are lack of dermatology services. As a result, the problem of uneven distribution does exist in dermatologists. Conclusion: It is useful to reduce the number of training dermatologists for the superabundant problem. For the patients’ convenience to see dermatologists, the department of health should solve the uneven geographic distribution problem. Suggestion: By the means of arranging the number of training dermatologists, the department of health has effectively mitigated the superabundant problem. Next, the government should fix the uneven geographic distribution problem by setting some lures or laying down policies to guarantee the patients’ right of seeing dermatologists
