背景與目的:疲勞(burnout)是身體與心理的疲憊與耗竭狀態,長期疲勞對身心具有相當負面的影響,醫院工作者嚴重過勞或過勞猝死事件迭有所聞,為瞭解醫院工作者的疲勞狀態,本研究以某公立醫院員工為對象進行職場疲勞評估,以為擬訂醫院職場健康促進計畫之實証依據。 研究方法:以配額抽樣法在中部地區之某公立醫院抽取醫師、護理、技術、行政人員進行問卷調查,得有效問卷405份,回收率96.4%。採國內研究者編製且信、效度均佳的量表為主要研究工具,含:中文版職場疲勞量表(個人疲勞、工作疲勞、服務對象疲勞、工作過度投入)、工作特質量表(工作心理負荷、工作控制、就業不安定性)、心理困擾量表、工作壓力。 研究結果:研究對象之個人疲勞 (男47.9分、女53.9分)、工作疲勞 (男42.0分、女50.4分)、每週工時(男50.4小時、女47.3小時)、自覺常常或總是工作壓力大的比率(男27.2%、女34.3%),均比國內針對台北市(2007)、全國(2004)受雇者疲勞調查結果為高,且高出甚多,可見本職場員工屬於高工作壓力、高疲勞的一群。護理人員在個人疲勞、工作疲勞、工作心理負荷、心理困擾程度的得分均顯著高於其它研究對象。在「整體職場疲勞」的迴歸模型中,達到顯著的疲勞預測因子為工作壓力、心理困擾程度、工作滿意度、工作心理負荷、就業不安定性,解釋力49.8%。 結論與建議:考慮性別與年齡差異,從改善員工心理困擾、工作心理負荷、自覺健康、工作滿意度面向切入,擬訂具醫院職場員工充權意識的健康促進計畫
Background and Purpose: Burnout is considered as physical and mental fatigue and exhaustion. Chronic burnout could present negative effects on body and mind. Hospital workers are often heard of serious burnout and sudden death caused by burnout. In order to understand the burnout status of hospital workers, this study aims to evaluate the workplace burnout of the staff in a public hospital for workplace health promotion project in hospitals. Research method:With Quota sampling, the doctors, nurses, technicians, and administrators in a public hospital in central Taiwan were proceeded questionnaire survey. Total 405 valid questionnaires were retrieved, with the retrieval rate 96.4%. The scales compiled by domestic researchers with good validity were applied as the research tools, including Workplace Burnout Scales – Chinese version (Personal Burnout, Work-related Burnout, Client-related Burnout, Over-Commitment), Job Characteristics scales (psychological work demands, job control, job insecurity), Psychological Disturbance scale (BSRS-5), and Work Stress. Research outcomes: The research results on personal burnout (males scored 47.9, females 53.9), work-related burnout (males scored 42.0, females 50.4), weekly working hours (males 50.4 hours, female 47.3), and perception of work stress (males 27.2%, females 34.3%) were much higher than the survey results of the employee burnout in Taipei City (2007) and nationwide (2004). Apparently, hospital workers were the ones who suffered from high work stress and high burnout. Nursing staffs appeared higher personal burnout, work-related burnout, work-related mental load, and psychological disturbance than other research subjects. Regarding the regression model of work-place burmout, the predictive factors of work stress, psychological disturbance (BSRS-5), work-related mental load, work satisfaction, and unstable employment achieved remarkable burnout, with the explanation 49.8%. Conclusions and Suggestions: In consideration of gender and age, a health promotion project with empowerment for hospital workers was drafted from the aspects of psychological disturbance of employees, work-related mental load, health awareness, and work satisfaction.
