The incidence of a eumorphic supernumerary incisor in the mandible is rare. Very few cases of 5 mandibular incisors have been reported in the literature, and no such cases have been reported from Taiwan yet. The case presented here is a rare incidence of a supernumerary incisor between the mandibular left and right deciduous canines. Cephalometric analysis further showed a slight skeletal class Ⅲ tendency. This tendency was likely caused by the anterior crossbite and thus is a pseudo-class Ⅲ case. We suggested removal of the unaligned tooth, orthodontic treatment, and then follow-up of the patient's development to see if further orthodontic treatment is needed,
