Rupture of Mediastinal Schwannoma in a Type Ⅱ Neurofibromatosis Patient Presenting with Traumatic Hemothorax
第二型神經纖維細胞瘤病患以創傷性血胸表現縱膈腔神經鞘瘤破裂：病例報告
摘要
縱膈腔內神經鞘瘤破裂造成的創傷性血胸非常少見。我們報告一個三十一歲男性病患，因腦部腫瘤曾接受手術而造成術後癲癇並規律地服藥。他因癲癇發作導致車禍而送到醫院，於胸管插入後表現出大量血胸合併生命跡象不穩定，後來以緊急開胸手術切除膈腔內破裂出血的神經鞘瘤而救治成功。以DNA序列分析出，在第二型纖維細胞瘤基因上發生染色體缺失造成移碼突變。

Traumatic hemothorax caused by a ruptured mediastinal schwannoma is rare. We report a 30-year-old male who had received a brain tumor operation. He suffered from postoperative convulsions and took medication regularly. He was sent to our hospital after a post-seizure traffic accident, and presented with massive left hemothorax which was found after insertion of chest tubes and unstable vital signs. The patient was successfully treated by emergent thoracotomy with resection of the ruptured schwannoma which was bleeding. A deletion mutation with frameshift of neurofibromatosis type 2 gene was detected by DNA sequencing.
