Xanthogranulomatous Pyelonephritis Complicated by Emphysematous Pyelonephritis: A Case Report and Review of the Literature

Xanthogranulomatous pyelonephritis (XGPN) is a rare, chronic pyelonephritis with yellowish multiple abscesses. Emphysematous pyelonephritis (EPN) is a rare, acute necrotizing and gas-forming renal infection. XGPN combined with EPN is very rarely seen. A 36-year-old female suffered from recurrent fever and left-side flank pain for one year and general malaise for one month. Renal ultrasonography showed a tumor-like lesion with a staghorn stone in the left kidney. Abdominal computed tomography showed a staghorn stone and gas-trapping in pyonephrosis, which converged on the collecting system in the left kidney. Pus culture from percutaneous catheter drainage yielded Proteus mirabilis and Viridans streptococcus. Therefore, XGPN complicated by EPN was diagnosed. Despite prolonged antibiotic use, pus discharge still persisted. Finally, the patient agreed to accept left nephrectomy, with a good surgical outcome. Renal pathology confirmed XGPN with superimposed acute infection.
