摘要

腦傷所造成的忽略症可能表現於不同的注意力參考架構之中。本研究回顧相關的文獻，並設計一視覺搜尋作業，旨在探討忽略症之注意力缺失的參考架構是以空間為基礎，或是以物體為基礎。本研究共包含三組受試者：忽略症病患組、無忽略症病患組和正常組。視覺搜尋作業的刺激為立體呈現的杯子，主要要求受試者判斷螢幕上是否出現有把手的杯子。實驗主要操弄下列兩個因子：目標杯子的空間位置（左側，中間和右側）和把手的形式（左側把手或右側把手）。實驗的結果主要發現，忽略症病患之正確率和反應時間的表現皆顯著較其他兩組的受試者為差：他們較容易忽略出現在腦傷對側空間的目標杯子；即使可以偵測到，其反應時間也顯著的增加，但與偵測出現在腦傷同側空間之目標杯子的反應時間並無顯著的差異，顯示忽略症病患的視覺注意力在腦傷同側的空間也有缺失。此外，雖然忽略症病患組的正確率只受到目標物呈現位置的影響；但在檢視個別病患的正確率後發現，部分忽略症病患的表現同時呈現以空間為基和以物體為基的忽略效果，支持忽略症狀包含多重的注意力參考架構。文中並討論本研究的結果在忽略症之臨床評估和治療的應用。

The purpose of this study was to investigate whether the frame of reference of attentional deficits underlying neglect was space-based or object-based in a visual search task. Participants were divided into 3 groups: stroke patients with neglect, stroke patients without neglect, and age-matched controls. All participants performed a visual search task to decide whether or not there was a target cup on the display. Two variables were manipulated to examine the frame of reference underlying neglect: one was the location in which the target was presented on the display; the other was the type of cup handle (left or right handle). The results showed that compared to other participants, neglect patients produced significantly more errors and longer reaction time for the contralateral left target. In particular, their reaction time for the ipsilateral right target was also significantly slower than those of normal participants, indicating attentional deficits on the ipsilateral hemispace. Moreover, only the location factor had significant effect on accuracy in neglect patients. However, analysis of the accuracy data of individual patient revealed that performance of several patients indicated both space-based and object-based neglect effect. These results demonstrate that neglect in these patients involved both space-based and object-based frames of reference. Implications of the findings for clinical evaluation and treatment of the neglect syndrome were discussed.
