工作強化介入模式及其成效之案例報告:職業導致椎間盤突出勞工

Effectiveness of Work-Hardening Case Report: A Worker with Occupation Induced HIVD

摘要


目的:探討職業導致椎間盤突出之勞工的工作強化介入模式及其成效。方法:個案為53歲男性,診斷為C2~6和L2~5椎間盤突出,從事網服印刷已30年。依據初次身體評估結果及新工作需要設計並執行工作強化訓練:為期六週、每週3次、每次約兩個小時。工作強化內容包括工作姿勢衛教、功能性體能訓練、職務再設計調整。結果:身體功能上,關節活動度與肌肉力量有明顯的增加。工作能力上,工作負重等級增加,而協調能力也明顯的進步。自我疼痛評量上,下背部的疼痛和酸麻情況已明顯減緩。工作強化結束時個案已在原職場擔任事務性為主工作。結論:研究顯示工作強化介入對於職業導致椎間盤突出的勞工有一定的成效,而加強工作強化與醫療資源的整合,及工作強化概念的推廣,可讓職傷勞工及早接受工作強化服務,並且儘早復工。

Purpose: This case study is to investigate the effects of a work hardening program designed for a worker with occupation-induced herniation of intervertical disk (HIVD). Method: A 53-year-old male subject was diagnosed as HIVD at the 2nd to 6th cervical spine and the 2nd to 5th Lumbar spine. He has worked as a printing press machine operator for thirty years. He participated in the two-hour work hardening program three times per week for six weeks. The program included the ergonomic education, physical and functional capacity training, job accommodation. Result: The improvements of the case in range of motion, muscle power, strength, and coordination were shown. The pain of the lower back and the tingle phenomenon of the extremities were obviously decreased. The case returned to the same work place performing different job tasks such as business administration after this program. Conclusion: The findings indicated that work hardening program is effective for a worker with occupation-induced HIVD. Integrating work hardening services and medical resources into a co-operated approach is recommended to help the injured workers to return to work as early as possible.
