Liver Subcapsular Hematoma as a Complication of Extracorporeal Shock Wave Lithotripsy

體外震波碎石併發肝臟被膜下血腫:病例報告及文獻回顧

摘要


體外震波碎石已經被廣泛的使用於處理泌尿道結石。肝臟被膜下血腫是此一手術極少見的併發症。我們報告一位51歲男性病人因為右側腎結石接受體外震波碎石,三天後因右上腹及右腰痛返回急診。於電腦斷層掃描發現肝臟被膜下血腫,住院後第三天因血腫破裂導致低血容性休克而進行緊急開腹探查及肝縫合。手術後並沒有進一步的出血。我們回顧文獻上有關此一併發症的報告並探討可能的原因。

Extracorporeal shock wave lithotripsy (ESWL) has been widely used for the management of urolithiasis. A fiver subcapsular hematoma is an extremely are complication of ESWL. We report a 51-year-old male pafenl who received ESWL for a right renal stone and experienced this complication. The pallent was sent to the emergency room with the chief complaint of right upper abdominal and flank pain 3 days after the ESWL. Anemia was noted at the same time. Computed tomography revealed a subcapsular hematoma of the liver. Rupture of the hematoma was noted with signs of hypovolemic shock on the third day of admission. An emergent exploratory laparotomy with hepatorrhaphy was performed. There was no further bleeding after the operation. The literatures on this complication is reviewed. and possible causes of the condition are discussed.
