摘要

背景：成功矯正創傷性鼻扭曲對於臨床手術，是一項極大的挑戰。本研究主要報告三軍總醫院耳鼻喉頭頸外科部實施開放式鼻成形術成功矯正創傷性鼻扭曲的經驗，並歸納整理一個簡單可依循的手術處理程序，有效改善鼻部功能及美觀。
方法：以回溯性分析自2001年8月l日至2004年06月31日間，計有92名創傷性鼻扭曲患者接受同一個手術處理程序，探討患者的手術適應症及手術效果。
結果：所有病患於全身麻醉下依此手術程序來矯正創傷性鼻扭曲，其中87名男性，5名女性，年齡分佈自15至53歲，平均年齡28歲，術後情況正常，無傷口感染或其他併發症產生。
結論：我們提出以開放式鼻成形術治療創傷性鼻扭曲的一個簡單有效的手術處理程序。這個程序適用在不同個體的解剖結構差異，可達到改善患者鼻塞及增進臉部美觀的效果。

BACKGROUND: Successful correction of traumatic twisted nose is a challenging clinical problem. The purpose of this study is to present our experiences of successful correction of traumatic twisted nose by open rhinoplasty in the Department of Otolaryngology-Head and Neck Surgery, Tri-Service General Hospital. We apply the surgical algorithm to correct traumatic twisted nose, achieving predictable, highly satisfactory outcomes.
METHODS: Ninety-two patients with traumatic twisted nose having undergone the same algorithm of surgical correction by open rhinoplasty between August 1, 2001 and June 1, 2004, were retrospectively reviewed.
RESULTS: All patients (87 males and 5 females) underwent open rhinoplasties under general anesthesia. Their ages ranged from 15 to 53 years with a mean age of 28 years. The postoperative conditions were uneventful, and there were no wound infections or other complications.
CONCLUSIONS: We propose a simple and optimal surgical algorithm using open rhinoplasty to correct traumatic twisted nose deformity. The algorithm, adaptable to individual variations in anatomic deformity, can yield consistently satisfactory functional and aesthetic results.
