摘要

週邊型T細胞淋巴瘤(Peripheral T-cell lymphoma)是一種較罕見的非何杰金氏淋巴瘤(non-Hodgkin's lymphoma)，其細胞組成是成熟T細胞(mature T-cell)。此疾病之臨床表現及細胞型態呈多樣性，所以在診斷及分類上困難度極高。本院於2002年8月經歷一名51歲女性，因快速進展的皮膚紅疹，合併頸部淋巴結腫塊而至本院求診，由於病人長期服用抗癲癇藥，起初懷疑是藥物過敏造成，住院治療無效且皮膚紅疹持續進展，經皮膚及淋巴結切片檢查始證實為週邊型T細胞淋巴瘤。所有的週邊型 T 細胞淋巴瘤皆是高分化度(high-grade)淋巴瘤，其病程大多進展快速，故積極的化學治療是絕對必要。本文就此病例提出報告，並探討其分類、組織型態、臨床表現及預後。

Peripheral T-cell lymphoma is a rare kind of non-Hodgkin's lymphoma, which is composed of mature T-cells. The diversity of the clinical manifestation and cell type makes the disease difficult to diagnose and classify. All kinds of peripheral T-cell lymphoma are high-grade, and most of the clinical courses progress rapidly. Once diagnosed, it can only be treated successfully by aggressive chemotherapy. This is a case report of a 51-year-old female patient, who was referred to our OPD on account of progressive erythematous skin rash and enlarged cervical lymph nodes. Since she had a history of seizure with regular medication, anti-convulsive medication was changed after consulting a neurologist. However, the skin rash still progressed, so biopsies of skin and a lymph node were performed. The pathologic study proved them to be peripheral T-cell lymphoma. The patient was thus able to be given timely chemotherapy and now enjoys a healthy life one year after the condition was diagnosed. We report such a case to discuss its classification, histology, clinical manifestation and prognosis and draw attention to the importance of erythematous rash with enlarged cervical lymph nodes for the early detection of peripheral T-cell lymphoma.
