利用細針抽吸後注射等量類固醇治療耳郭血腫

摘要

背景：耳郭血腫的治療一直都沒有標準化的做法，本文針對細針抽吸法進行修正處置，提供手術外的另一種治療選擇。
材料及方法：回溯性地收集自2006年1月1日至2007年11月30日間，所有罹患耳郭血腫造訪於本文作者之病患，排除1名手術後復發者後，共計14名(男12，女2)。年齡分布爲23~80歲，平均39.3歲，共計15耳（右11，左4），症狀發生時間爲3天～1個月。自病灶處抽吸出液體後，再注射入等量Triamcinolone Acetonide （Kenacort-A 10mg/mL，健亞生物科技股份有限公司，台灣），口服廣效性抗生素1天。1週後追蹤耳郭血腫消減的狀況，必要時再行一次處置，直到耳郭血腫完全癒合爲止。
結果：15患耳當中，初次處置時自病灶抽吸出液體的平均容積爲1.1mL。接受1次處置者7耳，接受2次處置者4耳，接受3次處置者4耳，平均恢復期爲1.8週。處置後，均無發生傷口感染。追蹤期爲1~18個月，均無發生血腫復發或耳郭變形的併發症。
結論：對於耳郭血腫之治療，除手術處置之外，建議可考慮重複以細針抽吸後注射等量類固醇處理。

BACKGROUND: As yet, there is no standard procedure for the treatment of othematoma. In this paper, all patients received a modified aspiration method instead of an open procedure.
MATERIALS AND METHODS: Patients suffering from othematoma were retrospectively collected from January 2006 to November 2007. After one patient with postsurgery recurrence was excluded, a total of fourteen patients (twelve males and two females) were enrolled. Their average age was aged 39.3 years, with a range from 23 years to 80 years. In total, there were fifteen ears made up of eleven right ears, and four left ears. The duration of symptom had ranged from three days to one month. The lesions were all aspirated by fine needle. Triamcinolone acetonide (Kenacort-A 10mg/mL, Genovate Biotechnology Co., Ltd, Taiwan) was immediately injected into the cavity using the same volume as that of the aspiration. In addition, a broad-spectrum antibiotic was given orally for one day. The patients were followed every week. The same procedures were performed again if residual othematoma was noted.
RESULTS: Initially, the average volume of fluid aspirated was 1.1mL. Among the fourteen patients, seven ears were treated once, four ears were treated twice and four ears were treated three times. The othematoma was found to subside over the following one to three weeks after treatment (1.8 weeks on average). None of the patients suffered from any wound infection. The follow-up period ranged from one to eighteen months and none of the patients suffered from any recurrence or auricular deformity.
CONCLUSION: In addition to an open de-roof evacuation, we recommend repeated
aspiration and equal-volume injection of Triamcinolone as a treatment for othematoma.
