Dropped or retained appendicolith is a rare complication after laparoscopic appendectomy, especially in ruptured appendicitis. Children is a vulnerable population that contained higher incidence of appendicolith and higher risks of ruptured appendicitis which accompany with appendicolith. We proposed a rare case of 13-year-old female who accepted laparoscopic appendectomy one month ago, and liver abscess with retained appendicolith was diagnosed finally. We only arranged percutaneous abscess drainage and broad-spectrum antibiotics treatment and the recurrence of abscess formation may occur in the future. This case reminds us that we should manage the appendicolith with interval time in the future.
