摘要

2020年新冠肺炎全球大流行，造成歐美多國急重症病患激增而醫療資源不足、被迫必須進行分配之倫理困境，相關醫學組織因此建立資源分配相關指引。本文以呼吸器分配為例，探討疫病大流行下之分流概念、建立分流指引的必要性、疫情期間稀少資源分配之倫理原則、建立分流指引的倫理問題，最後對我國目前因應策略提出省思。作者期望本文能為我國傳染病大流行期間之重症資源分配，提供學理基礎與初步討論，作為國家或醫學會擬訂相關政策指引之參考。

The 2020 Covid-19 pandemic has caused the surge of intensive care need and shortage of medical resources. Health professionals have encountered ethical dilemma of rationing and medical organizations hence established principles and guidelines for resource allocation. This paper uses ventilator as an example to explore the concept of triage under pandemic, the necessity of establishing triage guideline, ethical principles of scarce resources allocation, ethical problems arising from allocation, and reflects upon the current status of preparedness of Taiwan. It is hoped that through the authors' analysis, the theoretic foundation for discussion and the preparation for intensive care resource allocation of our government and medical societies can be promoted.
