中心位置咬合干擾對下顎運動之影響

The Influence of a Centric Occlusal Interference on Jaw Movement

摘要


本實驗之目的在於觀察中心位置咬合干擾對下顎運動的影響,以Hekimo index^1為測量的參考依據,並且使用感壓軟片作為咬點面積變化的記錄工具,期間並紀錄夜間咬肌肌電活性;實驗受測者共10位,五位男性、五位女性,平均年齡為28.1歲,咬合干擾高度為0.5mm,置入位置為下顎第一大臼齒中心窩上,時間為一個星期,實驗紀錄時段為咬合干擾置入後第一天,第二天,第七天,第十四天;實驗結果發現當中心位置咬合干擾出現時,會立即改變下顎運動模式,包括最大張口量、最大側方運動量、最大前突量,而咬肌活性也會出現立即的變化,之後會因為適應而出現恢復的趨勢。

This study was to evaluate the influence of a centric occlusal interference on jaw movement. The subject material consisted of five women and five men having a mean age of 28.1 years. Subjects were all in good general health and were without symptoms and signs from temporomandibular joint. A centric occlusal interference (composite resin of 0.5mm) was placed unilaterally on the central fossa of the lower first molar for 1 week. The jaw movement and masseter muscle activity were measured by Helkimo index and portable EMG system. The recorded data were obtained before and during day 1, day 2, day 7, and day 14 after insertion of the interference. There was a significant decrease in mandibular border movement and masseter muscle activity immediately after insertion of the interference on day 1. On day 14 the jaw movement and muscle activity were recovered to pre-experimental stage without causing any symptoms. It is apparent that occlusal interferences are associated with short-term clinical symptoms and signs but they won't have specific long-term detrimental effects.
