照護一位因痛風石引發壞死性筋膜炎病人之照護經驗
Critical Care Experience for Patient Suffering From Gout Stone and Necrotizing Fasciitis
摘要
本文探討一位痛風石患者併壞死性筋膜炎照護經驗。照護期間爲2008年10月17日至2008年10月20日，筆者使用Gordon十一項健康功能評估爲指引，並實際參與照護過程，運用觀察、會談方式收集資料，確立個案護理問題爲：一、現存性感染／與術後傷口感染有關、二、急性疼痛／與術後疼痛有關、三、知識缺失－對痛風之自我照護之相關之知識／與缺乏取得資訊有關、四、健康維護能力改變／缺乏對痛風飲食選擇之維護能力。經由主動關懷、傾聽病人的需求，進而提供個別性之照護措施，終於使得感染得以控制，並且讓病人的疼痛減輕。另外，藉由給於病人護理指導，以增進病人本身對痛風的認知，協助其獲得正確醫療資訊，增進對疾病的自我管理技巧，繼而改善病情。期望藉由此篇個案報告的撰寫，提供護理人員照護壞死性筋膜炎個案之參考。

This article described the critical care experience for patient suffering from Gout Stone and Necrotizing Fasciitis. The data were collected from observation and interview, through the period of nursing care was from Oct 17 to Oct 20, 2008, by using Gordon's 11 functional health assessment and participated in the nursing process. The confirmed nursing cases of the patients' problems included: 1. Infection, acute pain, knowlege deficit, and altered health maintenance diet modification and lack of health maintenance. The auth or provided individualized care to the patient through professional nursing knowledge, that allows the patient to develop the concept of self-management skill on disease prevention. We hope that this study would be a valuable clinical learning experience to share with the nurses that care for patients with necrotizing fasciitis.
