一位心臟手術後併發多重器官衰竭個案面臨預後不確定感之照護

Nursing Experience of a Post-cardiotomy Cardiogenic Shock Patient in the ICU

摘要

本文為一位中年女性於二尖瓣狹窄術後因右心室破裂合併心因性休克導致急性肝、腎等多重器官衰竭之加護護理經驗。照護期間自2018年5月25日至6月15日，藉由會談、溝通、身體評估及肢體語言等方式蒐集個案資料，依生理、心理、社會及發展、靈性的四大層面分析並確立個案有心輸出量減少、低效性呼吸型態、不確定感之主要三項健康問題。護理過程中積極維持個案血液動力學穩定、逐步脫離葉克膜及主動脈內氣球幫浦，並協助個案面對疾病、預後之不確定感，引導個案說出身心改變之想法，鼓勵個案參與治療過程並提供疾病相關資訊，增加對疾病的控制感，得以渡過此次危機，配合心臟復健介入在生理、心理及社會方面有良好的恢復。建議重症護理人員評估多重器官衰竭個案生理的健康問題，同時協助個案因應不確定感之威脅，增進對此病程之敏感度，適當的提供治療指引說明，幫助個案正向面對病程，以及早介入復健運動。藉由此護理經驗，期望提供護理人員在照護類似個案之參考與運用。

This article explores the experience of a multiple organ failure patient suffering from Post-cardiotomy cardiogenic shock (PCCS) in the intensive care unit over a period of three weeks. The patient received intra-aortic balloon pump, extracorporeal membrane oxygenation, and continuous veno-venous hemofiltration treatment. During the hospitalization period, assessment of the patient's physical, psychological, social, and spiritual status was completed through observation, assessment, interview, and written communication. The author had discovered that the patient had defect in decreased cardiac output, ineffective airway clearance, uncertainty, body fluid overload, risk for hospital acquired infection, and activity intolerance. We provided closely monitored the hemodynamics and supportive environment to help the patient survive from cardiogenic shock and uncertainty levels. The patient successfully survived the acute stage and was relieved of uncertainty. It is hoped that the holistic care experience could provide the medical teams reference for further clinic care.
