精神疾病是屬於長期慢性化疾病,因認知功能障礙,會影響其自我照顧及社會適應能力,故協助病人因應疾病與提升社會生活處理能力是必要的,本專案以某慢性精神科病房住院病人為對象,現況分析發現社會生活功能中度以上者僅22.22%,主因為:醫療團隊未擬定階段性住院目標及社會生活功能復健計劃、病人不知道參加復健活動目的、缺乏社會化訓練場域及缺乏參與活動動機。解決方案:整合跨團隊訂定社會生活功能評估作業標準、制定個人化復健活動課程作業、建立社區融合活動方案、建立實質獎勵制度。實施後,病人社會生活功能中度以上者由改善前的22.22%提升至41.27%,顯示此專案推行確實能提升病人社會生活功能,使精神病人能早日回歸社區生活。
Mental illness is considered a long-term chromic disease. Patients’ ability for self-care and social adaptability are reduced to due cognitive dysfunction. Therefore, it is necessary to assist patients to improve their social and life skills. This project focused on a group of patients in a chronic psychiatric ward. Of the patients analyzed only 22.22% were found to have social life skills at or above an average level. The primary reasons for this are as follows. The medical team did not setup periodic goals and develop a social life skills improvement plan. Patients were unclear about the purpose of joining the improvement plan. They had limited training in social and life skills and lacked motivation. In terms of resolution, it is important to integrate various medical professional teams to launch standardized social life skills evaluation, set up personalized recovery activity coursework, establish a community integration plan, and create a reward system. After implementation of the proposed solution, patients at or above average social life skills increased to 41.27% from the previous mark of 22.22%. These results show that this project can improve patients’ social life skills, leading them to earlier community reconnection.
